March has been a busy month for the Hospital Transformation Program (HTP) and the Community and Health Neighborhood Engagement (CHNE) process! I have been lucky enough to join many of your CHNE meetings, either in person or via conference call, and it's exciting to see the teamwork happening across the state. While communities are doing their part, hospital workgroups and Department staff continue to have weekly meetings to develop and refine the measures list for the program.
 
In addition, the Department had an initial conversation with the Centers for Medicare & Medicaid Services (CMS) to review the HTP Concept Paper and establish further programmatic discussions. We are committed to keeping stakeholders updated as the waiver process progresses.

Community and Health Neighborhood Engagement Process

Congratulations to East Morgan County Hospital and their staff, who had the Fort Morgan Times join them at their CHNE meeting! In the spirit of true collaboration, hospital leadership asked meeting attendees and community organizations, "How can we support you?" Keep up the great work!
 
Since the beginning of the year, the Department has participated in 14 community meetings across the state, including a metro Denver meeting involving six Denver area hospitals and the Colorado Community Health Network Board of Directors. The level of engagement taking place across the state is truly incredible. We are really seeing a culture of engagement take hold with the CHNE process and it is amazing to be a part of.

Midpoint Reports

One month to go! 

Due April 19, 2019 , hospitals will report  on the progress made in the first phase of the CHNE process in their Midpoint Reports, including updates on community engagement and results of their environmental scan of community characteristics, needs, and resources. These reports will be made available to the public through the HTP website. The Midpoint Report template can be found on the HTP website, as well as a link to the Midpoint Report training presentation . An audio file of the presentation is available.
 
We have provided a number of data resources to support this process. In addition to the data resource links on the HTP website, including data from the Colorado Department of Public Health & Environment and the Colorado Health Institute, the Department sent each hospital a series of Medicaid data reports.
 
Midpoint Reports are a good tool to keep in mind as CHNE meetings are taking place. When convening, hospitals and stakeholders should come prepared with information about the community, including the results of recently completed community needs assessments. Community needs and resources should be compared to HTP priorities to begin identifying opportunities for initiatives and partnerships.
 
Midpoint Reports must be submitted in .pdf form with all supporting documentation included in one document via e-mail by April 19, 2019 at 5:00 p.m. to the Colorado HTP email address COHTP@state.co.us. Reports received after this deadline will not be considered. Additional CHNE resources can be found on the HTP website.

HTP staff would love the opportunity to participate in your community meetings and learn more about your CHNE process! Eventually, we'd like to create a comprehensive list of all community meetings for historical and transparency purposes.

The next deliverable for the CHNE process is the Midpoint Report due on April 19, 2019.
