CHCAMS Spring Clinical Conference
CHCAMS hosted its Spring Clinical Conference Friday, March 8 th  at the Hilton Garden Inn, Flowood. The theme: “Systems of Care: Best Practices for Clinical Integration” laid the foundation for our ongoing trainings to support CHC efforts for behavioral health integration as a seamless process in the provision of primary care. The specific topics addressed the coordination of both internal and external systems for patient-centered care. The 50 participants—representing 13 Mississippi Community Health Centers (CHC)—were a multi-disciplinary mix including but not limited to CHC leadership, clinicians, nurses, social workers and informatics staff.

Violence within the family becomes a way of life, and is the breeding ground for other social problems, such as substance abuse, mental instability, and violent crimes of all types. Trauma Informed Care Across the Addition Continuum
A person's experience of trauma impacts every area of functioning,… physical, mental, behavioral, and social. Implications for trauma-informed care means treating a whole person, taking into account past trauma and the resulting coping mechanisms when attempting to understand the patient’s behaviors while empowering them to make their own decisions.

Recovery Oriented Systems of Care
ROSC coordinates services and support that build on the strengths and resilience of those with or at risk of alcohol to improve quality of life.

Motivation Interviewing
It is a directive, client-centered counseling style for eliciting behavior change by helping clients to explore and resolve ambivalence. OARS is a key to elicit focused and goal-directed conversation.  O – Open-ended questions; A – Affirmations; R – Reflective listening; S - Summaries

The health center board adopts, evaluates at least once every three years, and, as needed, approves updates to policies in or that support the following areas (emphasis on #3):

  1. Sliding Fee Discount Program
  2. Quality Improvement/Assurance
  3. Billing and Collections (those that address the waiving or reducing of amounts owed by patients due to inability to pay, and if applicable those that limit or deny services due to refusal to pay)
  4. Financial management and accounting systems and personnel

Medicaid Proposed Significant Changes in Support of Services for
Targeted Case Management (TCM)
 Effective January 1, 2019
State Plan Amendment (SPA) 19-0001 Targeted Case Management (TCM) for Beneficiaries With Intellectual and/or Developmental Disabilities (IDD) in Community-Based Settings, with a proposed effective date of January 1, 2019, has been submitted to allow the Division of Medicaid to:
  1. Add Autism Spectrum Disorder (ASD) as a covered diagnosis,
  2. Remove the needs-based criteria,
  3. Revise the qualifications and training requirements for Targeted Case Managers, and
Revise the reimbursement for TCM for beneficiaries with IDD in community-based settings from $14.88 per fifteen (15) minute unit to a rate of $151.01 per month.
Final Title X regulation released by HHS February 2019
 
A summary of the ruling focuses on compliance with the underlying Title X statute. It also provides clarity between permissible Title X activities and impermissible ones by requiring clear financial and physical separation for Title X funded programs from programs and facilities where abortion is a method of family planning. Read the facts  here .

Y our health center may be eligible to apply for one of five $200,000 BD Helping Build Healthy Communities Awards (Coastal featured here in their honor as a previous recipient)!
BD Helping Build Healthy Communities is a multi-year initiative that expands access and improves care for underserved and vulnerable populations throughout the United States.
The initiative, funded by BD and implemented together with Direct Relief and the National Association of Community Health Centers (NACHC), provides awards to community health centers with innovation programs in Medication Therapy Management. The funding is to support programs that operate over a two-year period.
BD Helping Build Healthy Communities aims to identify, award, and share successful community health innovations with other safety net providers, officials, and the public to generate awareness of health centers’ unique ability to address community needs. Up to five (5) Awards of $200,000 each will be made in 2019 for Medication Therapy Management Programs.
 
Eligibility Criteria
  • Organizations from all 50 states, D.C., Puerto Rico, and the U.S. Virgin Islands
  • Applicants must be Direct Relief partners, to become a Direct Relief partner, contact usaprograms@directrelief.org
  • Applicants must be federally-qualified health centers, or look-alikes, and a NACHC member
  • Regional health center consortia and statewide associations are not eligible to apply
  • Past Award winners may reapply if they have not received funding the past two award cycles

N otice of Funding Opportunity for Rural Providers: Small Health Care Provider Quality Improvement Program HRSA recently released a notice of funding opportunity that will support up to 32 public or private nonprofit health care providers located in rural areas to improve patient care in their communities. HRSA expects to invest approximately $6.4 million for this program, which aims to improve the quality of rural health care through an evidence-based approach to primary care that has proven effective in rural areas.
Previously funded organizations under this program designed and carried out projects focused on coordinated care for patients, chronic disease management, integrated care delivery systems, and reduced admissions for emergency rooms and hospitals, among other issues for rural providers. Objectives for this program, which is competitive every three years, include improved health outcomes, better engagement of patients and their caregivers, and continuous improvement to chronic disease management. Grantees also are encouraged to implement quality improvement activities that measure value by outcomes, establish and maintain PCMHs, and integrate behavioral health into the primary care setting.
