For help in completing your tax forms, download a spreadsheet of enrollment data from your My Account home page at QualChoice.com. We encourage you to review the information carefully and let us know of any changes or corrections needed. For questions or concerns, contact your Account Service Representative.
Reporting Health Coverage on Taxes
2018 health coverage must be reported on federal tax returns. Review which forms members will receive from QualChoice for each of these plan types:
  • Employer Group
  • IQChoice Individual/Family
  • Arkansas Works
  • Health Insurance Marketplace
