Thank you to all the facilities that have completed and returned the facility PDSA form. The Network appreciates all efforts to promote and increase the utilization of home modalities.
The Network has received multiple question about data submission for this quality improvement activity. All data is being supplied to the Network by the National Coordinating Center (NCC).During the this activity the Network is tracking and monitoring two sets of data.
The first is the facilities overall home modality training rate. The NCC is using the CROWNWeb field " Date Training Began" to identify the number of patients a facility has trained in a given month. The number of patient(s) who begin training in a particular month, becomes the facility's numerator. The denominator is the facility's in-center patient census minus the individual(s) who are training for home. It has come to the Network's attention that this data is not always batched into CROWNWeb correctly and may need to be manually entered. Below is a picture of the field that the NCC is using to collect the data for the facility numerator. Please review CROWNWeb to ensure that all training start dates are accurate.
The second data set of data the Network is monitoring patient progression through the seven step. CMS developed a seven stop process to monitor how in-center hemo dialysis patient are educated on home modality and the process that is needed to moved form in-center to home dialysis. The seven steps are as followed:
- Patient interest in home modality
- Educational session to determine the patient's preference of home modality
- Patient suitability for home modality determined by nephrologist, with expertise in home dialysis therapy
- Assessment for appropriate access placement
- Placement of appropriate access
- Patient accepted for home modality training
- Patient begins home modality training
Facilities that are part of a Large Dialysis Organization (LDO) your corporation had provided the NCC data for the seven steps. The Network's Small Dialysis Organization (SDO), there will be a REDCap surveys provided the facility to manually enter patients into the seven steps.
