As 2018 comes to a close, we express gratitude and appreciation for all the North Jersey Health Collaborative (NJHC) partners and funding partners.  We are inspired by our partners dedication and commitment to our collective work as we continue to share data and resources, assess the health-related needs of our communities, and develop targeted plans and strategies to create healthy communities ~ healthy people throughout northern New Jersey.   


Highlights from 2018 
  • More than 100 Partners and 26 Funding Partners are participating in the NJHC.
  • Released our first Annual Report 
  • Developed our first three-year NJHC Strategic Plan 
  • Formed new relationships and worked together in new ways to address a host of health-related issues including diabetes, obesity, food access and mental health. 
  • Forward Franklin entered its second year and implemented a mini-grant program to improve public spaces in Franklin through a community revitalization fund. Learn more here.  
  • Horizon Foundation Grant "Morris County Diabetes Intervention Pilot" is underway with 12 graduates and an additional 37 participants. 
  • Morristown United for Healthy Living Coalition enters its fourth year and has recruited more than 20 street captains to increase resident engagement. Read more about this here.
  • Coordinated more than 100 Collaborative meetings. 
  • Offered three professional development opportunities to build partner capacity.
  • Began our second shared Community Health Needs Assessment and conducted our first Community Voice Survey, collecting more than 4,000 responses.
  • NJHC celebrated its 5th Anniversary in November!
We look forward to continuing our work together in 2019.

At the January meetings the county partners will have the opportunity to refine, clarify and focus in on which strategies will be prioritized for 2019. Be sure to invite the potential partners you identified at the October meetings to join us in January.
