FAMILY SERVICE LEAGUE OPENS 24 HOUR CRISIS CARE CENTER FOR SUFFOLK COUNTY RESIDENTS

On March 1, 2019, the Family Service League held a ribbon-cutting celebration for a first of its kind on Long Island crisis care center.  The Diagnostic, Assessment and Stabilization Hub (DASH) offers 24 hour, 7 day a week, year round crisis care at a community based site for children over 5 years of age, teens, adults and families.  The program is available to Suffolk residents who struggle with mental health issues and/or substance use disorders.  DASH is located at 90 Adams Avenue, Hauppauge, NY.

GURWIN JEWISH NURSING & REHABILITATION CENTER RECEIVES NYS GRANT
For Advanced Care Unit for Medically Complex Patients
Stuart B. Almer, President & CEO, Gurwin Family of Healthcare Services, cuts the ribbon for the new Advanced Care Unit with Suffolk County Legislator Susan Berland and Assemblyman Andrew Raia

Gurwin Jewish Nursing & Rehabilitation Center is the recipient of a New York State grant to help fully fund its new, 12-bed Advanced Care Unit for patients with complex medical needs.  

The specialized unit provides a higher level of care for fragile, clinically complex patients, which at one time was only available in the hospital setting.  Gurwin is one of the few skilled nursing facilities on Long Island to offer care for residents with serious, multiple co-morbidities in a dedicated unit.

VITAL SERVICES FOR EAST END CHILDREN AND ADULTS

Family Service League's new Riverhead office is located at 400 West Main Street, Suite 300, Riverhead, NY.  The services offered include:

Family Recovery Center
Confidential substance use disorder treatment to adolescents and adults seeking services voluntarily and by mandate.  Treatment is also provided to family and significant others whose lives have been affected by addiction.

Mental Health Clinic
Broad spectrum of programs, designed for children, teens, and adults, to treat all aspects of psychiatric illness and emotional distress.

Community Advocacy Information and Referral
Free walk-in or phone-in help for anyone experiencing a variety of needs, such as temporary emergencies involving insufficient food, utility shutoffs, potential loss of housing or other essential resources.   Staff offers community advocacy, supportive counseling and linkage to other programs.

STONY BROOK CANCER CENTER SPONSORING CANCERWISE CAFE

Learn about the latest advances in screening, prevention, diagnosis, management and treatment of several types of cancers. These include breast, lung, colorectal, blood, head and neck, gastrointestinal, gynecologic, prostate and skin cancers.

Educational displays will provide information on services available at Stony Brook Cancer Center.   Resources from community organizations will also be available.   The event is free and includes a light lunch.  Appropriate for those 16 years or older.

JOIN SCC FOR CULTURAL COMPETENCY AND HEALTH LITERACY MASTER TRAINING
March 26, 2019

The communities we serve are growing in cultural and linguistic diversity. There is a need to inspire cultural change within organizations. 

On March 26, 2019, 8:30 am to 5:00 pm, SCC will be holding Cultural Competency and Health Literacy (CCHL) Master Training, an interactive program in which participants will have hands-on learning experience to build better communities. 
 
Post-training, Master Trainers will receive a certificate of completion and will be able to provide training sessions.  Master Trainers will be able to facilitate two different trainings: (1) 2 hour CCHL training sessions for workforce professionals and (2) 7.5 hour master training sessions for future master-trainers.
 
Participants must apply.  Please note, completing the application does not confirm attendance to this program. Upon completing the application, you will receive follow up from a CCHL lead with next steps.    For more information on the CCHL program  click here.

NYS DOH ANNOUNCEMENTS

2019 VBP MLTC, PACE, MAP AND FIDA QUALITY MEASURE SETS 
The 2019 VBP MLTC, PACE, MAP and FIDA Quality Measure Sets are available on the DOH website, Value Based Payment Resources Library webpage.

The 2019 VBP arrangement quality measure sets were created in collaboration with the Managed Long Term Care (MLTC) Clinical Advisory Group (CAG), a Sub-team empaneled by the CAG to discuss VBP approaches and quality measures for Partial, Medicaid Advantage Plus (MAP), Fully Integrated Duals Advantage (FIDA), Programs of All-Inclusive Care for the Elderly (PACE) and the New York State (NYS) Value Based Payment (VBP) Workgroup. The measure sets are closely aligned with measure sets used in the NYS Department of Health MLTC Quality Incentive to reward MLTC plans for performance on quality measures. The VBP Quality Measure Sets are intended to encourage providers to meet high standards of patient-centered care across multiple care settings for members of MLTC plans.

The following 2019 measure sets are now available on the VBP website:

MACRA WEBINAR SLIDES AVAILABLE
The slides from the March 1, 2019, Medicaid Access and CHIP Reauthorization Act of 2015 (MACRA) and NYS Value Based Payment (VBP) Alignment webinar are now available on the DOH website, under the MACRA Alignment tab.  For questions email VBP@health.ny.gov. 

REQUEST FOR APPLICATIONS - OLMSTEAD HOUSING SUBSIDY
The New York State Department of Health (NYSDOH), Office of Health Insurance Programs (OHIP) and the Medicaid Redesign Team (MRT) announced the release of the Olmstead Housing Subsidy Request for Applications (RFA).  NYSDOH is seeking applications from community-based organizations to provide rental subsidies and non-medical services to Medicaid members who are looking to transition from a Skilled Nursing Facility (SNF) and move back into the community.  Participants in the program must be on Medicaid, homeless, currently reside in a SNF, have spent at least 120 consecutive days in a SNF over the most recent two-year period, and can live safely in the community.  

The Olmstead Housing Subsidy is a prior funded program that is in effect from 8/1/2016-10/31/2019.  NYSDOH is seeking to fund up to one project to provide rental subsidies and supportive housing services statewide.  The anticipated amount of funding is $35,500,000 over five years.  For more information or to apply to the RFA click here.  A copy of the RFA can be
