Stakeholder Feedback Requested on Model of Care Recommendations for Multipurpose Senior Services Program
In preparation for the transition of Multipurpose Senior Services Program (MSSP) sites to become a fully integrated Medi-Cal managed care benefit, the Department of Health Care Services (DHCS) and the California Department of Aging (CDA) have convened a Model of Care Workgroup composed of the two agencies, MSSP sites, and managed care plans. The workgroup's goal is to develop recommendations for a model of care for the new Home and Community-Based Services Care Planning and Management benefit that will take the place of the MSSP program.

DHCS and CDA are seeking public comment on the workgroup's first set of recommendations. The public comment period will be open for the month of September, with comments due by close of business on Monday, October 1, 2018 to [email protected].
Cal MediConnect Agent/Broker Pilot: Plan Selection, Readiness, and Data Reporting Overview
Last week, DHCS announced that L.A. Care Health Plan is the only Cal MediConnect plan which applied and qualifies for the Cal MediConnect Agent/Broker Pilot. Release of Cal MediConnect est Practices Summary for Long-Term Services and Supports

One key focus of Cal MediConnect (CMC) - as well as the broader Coordinated Care Initiative (CCI) - is to better integrate long-term services and supports (LTSS) for Medicare and Medi-Cal beneficiaries. Several years into the program, evaluation findings indicate that, while CMC provides a valuable new pathway for serving dual eligibles, more work can be done to:
  • Connect members to LTSS services; and
  • Better integrate and coordinate those services with the more traditional medical benefits that health plans offer.

8/16 - Caregivers and beneficiaries learned about Cal MediConnect from state outreach staff during the Inland Behavioral & Health Center's Community Festival in San Bernardino.



San Diego County

8/17 - State outreach staff spoke with beneficiaries and shared CCI materials while hosting a resource table at the Mira Mesa Senior Center Resource Fair in San Diego.
Santa Clara County


8/15 - A state outreach specialist shared CCI information in a presentation to the monthly caregivers' support group at Live Oak Adult Day Center in San Jose.
