The following Request for Information has been released by PA Department of Human Services Secretary Teresa Miller.

Dear valued stakeholder,
 
The Department of Human Services (DHS) recently released a Request for Information (RFI) to explore existing individual or family needs assessments, methods of connecting individuals and families to community resources, and models for providing whole-person or whole-family case management. Our goal is to build a system that addresses each family’s needs and amplifies the work of health care providers and community organizations. 
 
The department recognizes the frustration individuals and families may experience while trying to access much needed services in a timely manner, when receiving case management from multiple agencies. We want to make it easier for individuals to obtain meaningful information and access to the services they need to achieve overall wellbeing, positive health outcomes, and financial self-sufficiency.
 
DHS hopes to learn more about existing individual or family needs assessments, methods of connecting individuals and families to community resources, and models for providing whole-person or whole-family case management. If your organization has experience in any of the areas outlined in this RFI, I invite you to respond by January 18, 2019 by visiting the Pennsylvania eMarketplace website .
 
I also invite you to share this request with other community partners in your network who may be interested in providing input. I know there are many competing priorities, but I hope you or someone from your organization can take some time to respond – even if you are unable to answer every question. This information will inform future efforts to develop and improve service coordination and access for individuals enrolled in DHS programs, as well as all Pennsylvanians.
