Dr. Stella Safo, Senior Medical Director of Clinical Transformation, Population Health, joins Dr. Robert Fields, Chief Medical Officer, in the latest episode of the Mount Sinai Health Partners podcast to discuss transforming primary care for population health. Motivated by the connection between social justice and primary care, Dr. Safo’s work addresses the question of how to survive in primary care in a way that’s sustainable. She and Dr. Fields discuss some of the stresses of primary care, goals of clinical transformation, and physician burnout. She also shares her vision on the art of medicine.

Fulfill Your 2018 CIN Requirements Today 

You may have heard from your department/division leadership regarding your outstanding requirements. To fulfill the mandatory 2018 Clinical Integration Program Requirements, you may need to a complete a few videos or PowerPoint modules on Mount Sinai’s Clinical Integration Learning Center (CILC), which is available through  PEAK , Mount Sinai’s education portal. Below are the 2018 requirements:
 
  • Care Coordination (First year providers only): Sign MSHP PCP-specialist compact describing a framework for better communication and safe transition of care between primary care and specialty providers. Please access the MSHP Care Compact Attestation via the Clinical Integration Learning Center, which is available from PEAK.
  • Program Engagement (First year providers only): Providers must complete the following tasks: View on CILC through PEAK and attest completion for the following courses on the Clinical Integration Learning Center: Clinical Integration 101, Clinical Integration 201, and Clinical Documentation Quality Improvement (CDQI). 
  • Program Education (Primary Care Physicians only): Attend your quarterly on-site population health meetings. 
  • Privacy Training: Complete the Mount Sinai Health Partners online 2018 Health Insurance Portability and Accountability Act (HIPAA) privacy training module on CILC through PEAK.

Study on Using Primary Care Wraparound Services to Reduce Hospitalizations

In this month’s issue of Health Affairs, Joshua Vest and colleagues report a 7% reduction in hospitalizations and 5% reduction in ED use associated with the receipt of primary care wraparound services (Dietitian, Social Work, Patient Navigation, Behavioral Health, Asthma Education) in an urban safety net health system in Indianapolis. Urgent Care or Emergency Room?

Urgent care centers serve non-life threatening but immediate healthcare needs. Emergency Department (ED) use should be reserved for life-threatening emergencies that need the highest level of care. To reduce unnecessary, expensive ED visits please help educate your patients on when to visit the ED versus urgent care and encourage same-day appointments in your practice.

MSHS Employee Health Benefit Plan Top Tier Transition
Effective Jan 1, 2019

The administrator of Mount Sinai Health System’s (MSHS) Employee Health Benefit Plan will transition from Empire BlueCross BlueShield to UMR, a division of United Healthcare effective January 1, 2019. Providers participating in the Top Tier network will automatically be migrated and loaded in UMR’s system for Mount Sinai employees and their eligible dependents only.
 
The “Top Tier” is a network of Mount Sinai Health System (MSHS) hospitals and employed and community-based health care professionals that provide health care services to MSHS faculty and staff, and their eligible dependents, at a level of coverage that has the least out-of-pocket cost to MSHS employees.

Bright Health Webinars

Last month, Mount Sinai launched a co-branded Medicare Advantage Insurance Plan with Bright Health built around Mount Sinai's hospitals and physicians. As a member of MSHP’s CIN, your practice is automatically enrolled in this fee-for-service arrangement that is effective on January 1, 2019.

Bright Health has scheduled webinars to provide you more information on how to do business with them. Register for a webinar by clicking the date and time you'd like to attend below so you can hit the ground running on January 1.
