San Francisco Interfaith Council Survey
on Programs and Services for Aging Adults
and Persons with Disabilities

Communities of faith are traditionally hubs of gathering for aging adults and persons with disabilities where these individuals feel safe, valued and are able to overcome social isolation. The San Francisco Interfaith Council, working in collaboration with our cross-sector partners, the SF Department of Aging and Adult Services, SF In-Home Supportive Services, Coro Northern California, with foundation support from the Metta Fund, invites you to complete a short survey regarding programs and services offered to aging adults and persons with disabilities.

The survey will  take less than 10 minutes to complete. It is designed to reveal critical information to policy makers regarding existing programs and services being provided, the degree to which congregation leaders are aware of and are able to assist their congregants in accessing public services, as well as offering a portal to express observations and concerns.
