Senior Resources to create Medicare Advantage Plan for Special Needs through Michigan Health Endowment Fund grant

Senior Resources of West Michigan recently was awarded a grant from the Michigan Health Endowment Fund to develop a medical provider program providing medical and case management services, while working to become a Medicare Advantage Plan for Special Needs (SNP).

The program will allow for better integration of home and community-based services with the provision of medical care to address social determinants of health, resulting in better patient outcomes. Goals include lower emergency department utilization, reduced hospitalizations, and increased longevity for participants.

The Michigan Health Endowment Fund works to improve the health and wellness of Michigan residents and reduce the cost of healthcare, with a special focus on children and seniors.

Senior Resources joins national campaign to raise awareness of local home modifications programs

Most older adults want to continue living in their homes as they age. Yet many homes often need to be adapted to meet the changing mobility, sensory and cognitive needs of the older adults who live in them. As a result, older adults and caregivers often turn to the Aging Network for assistance with modifying their homes to meet these changing needs. Sometimes, minor home modifications may be enough to keep an older adult in their own home.

As a result of these changes, many older adults and their caregivers will turn to local organizations for support with updating their homes to meet their evolving mobility needs, which can be a daunting process. This is the reason Senior Resources is participating in Home for the Holidays, an annual public education campaign led by the Eldercare Locator, the National Association of Area Agencies on Aging, and the U.S. Administration for Community Living, this year in partnership with the University of Southern California Leonard Davis School of Gerontology.

Because older adults and their family caregivers may not know where to start on the home modification process, Senior Resources is participating in the Home for the Holidays campaign to ensure older adults in our community have the tools and resources they need as they update their homes-and continue living in the community. Senior Resources can connect older adults and their caregivers to services and programs that can support them as they modify their homes for successful aging.

For more information about the campaign and other local resources available for older adults and their caregivers, contact CALL 2-1-1, or Senior Resources' options counselors at 231-733-3585. We will soon have brochures with some helpful tips for home modifications.
