Each year advisors often have to have students complete Medical and Code of Conduct forms three different times for Ohio HOSA sponsored events (Fall Leadership Conference; Regional Competitive Events; and State Leadership Conference). In an effort to reduce advisor workload, we are encouraging you to have each member complete both forms and then the chapter mail the forms to the State Advisor.
In turn, Ohio HOSA will maintain the forms and have them available at each event. Therefore, an advisor would not need to submit copies at each Ohio HOSA sponsored event.
To accomplish this advisors may wish to have students complete these forms the first several weeks of school. You may download and print the forms by clicking on the below link.

Advisors are still encouraged to bring a copy of the form for their use in case of need and also the student should have a coy of their medical form with them at all tmes during thei respective event.

This is not required -- you may choose to bring the appropriate forms day of the event. Additional forms for new members may be mailed in as you affiliate members.
