It is not uncommon to have some erroneous information listed in MyCommunity Profile, including office addresses, phone numbers, email addresses, and taxonomy. When I verified my information in MyCommunity Profile, I was still listed as a practicing Gastroenterologist at old addresses. I was listed at 6-7 addresses and a few of them were completely wrong. It took me about 10 minutes to go through the information and make online corrections. Within 2-3 days of making updates, I received a verification email from Rochester RHIO that the information I had provided had been validated.
We are trying to reach all RHIO participating physician’s offices, urging them to review their profile and make corrections. Up-to-date community physician profiles will go long way for improving transitional care communication and supporting better patient care overall.
Finally, I want to thank all the practices that are contributing complete C-CDAs through their vendors to Rochester RHIO. Out of 77 practices and 340 providers, only six practices and 19 providers have minor gaps in data quality like Race and Ethnicity or Results (Labs). Thank you all for the overwhelming response and rectification of the deficiencies I have pointed out.
I am still collecting the
three most important quality measures you consider crucial in your practice for CMS reporting purposes, even if you are not reporting, or reporting through your vendor or IPA. This is to make sure that Rochester RHIO is able to provide health care information to you for quality matrix reporting.
Send me an email about your three most important quality measures. Email
