On October 2nd, 2018, Staten Island Performing Provider System will sponsor its first Population Health Symposium. Staten Island PPS is distributing the Call for Posters (below) to those who wish to have their work displayed at the symposium.

The Symposium provides a forum for disseminating research findings to frontline healthcare providers. The poster session allows participants to discuss noteworthy projects that have been piloted at local health care organizations. A Symposium Planning Committee will review abstract submissions for the poster session, and accepted presenters are notified by September 7th, 2018.

NEW Certified Recovery Peer Advocate (CRPA) Training Program!

In collaboration with the College of Staten Island (CSI), Staten Island PPS is pleased to launch the new Certified Recovery Peer Advocate (CRPA) Training Program. This program is a core strategy of the PPS' mission to help expand the Behavioral Health Peer Workforce on Staten Island and improve behavioral health outcomes. The CRPA program incorporates significant input from multiple PPS partners and peer employers to expand on the core CRPA training and focus on preparing participants for employment and expectations of different work settings, improving communication at work, building soft-skills, and preparing participants for the Peer Recovery Credentialing Exam. Scholarships are available from SI PPS and 1199 SEIU tuition benefits for eligible 1199 employees. The program also offers post-program assistance with employment and continued education. The CRPA training program will begin August 20th, 2018 through October 19th, 2018. Substance Use Disorder Treatm ent Providers and Recovery Centers are encouraged to nominate individuals to apply by today, July 31st. 



How the Community Health Worker Program Gave New Hope to One Student

"After working at Richmond University Medical Center for over 25 years, my position as a senior tech was eliminated. Although it was a difficult time in my life, I knew that the job elimination would also provide me a second chance at finding a rewarding career where I could make an impact on the Staten Island community. I soon found an opening in the West Brighton Clinic Behavioral Health Department as a Community Health Worker. In order to attain the skills and knowledge needed to perform in the role, I enrolled in CHW training at the College of Staten Island with the financial support of Staten Island Performing Provider System. Now that I have completed the [CHW] program, I feel I am better prepared to help and contribute what I have learned in the program about DSRIP, motivational speaking, chronic illness such as diabetes, hypertension, addiction, and a variety of other topics, including the critical cultured shift to patient- centered services. As health policy, research and practice are becoming increasingly focused on improving the health of populations and addressing social determinants of health, Community Health Workers may be just what the doctor ordered."

SI CARES at Project Hospitality
SI CARES Health Coach Oluwakemi Adeoso, from Project Hospitality, enrolled Mr. JH in May 2018.  Upon doing an intake and assessment, the Health Coach determined that Mr. JH was in need of assistance to help him get to a doctor. During the relationship built between Mr. JH and Health Coach Oluwakemi Adeoso, the client informed his Health Coach that he had not been to a physician in years. Mr. JH explained that going to a doctor was a fearful experience and as a result he would avoid doctor's visits altogether. Mr. JH is aware of his diagnosis of hepatitis, but did not follow through to get the appropriate care due to his own fear of doctors. Mr. JH's biggest concern was fear of bad news related to his health. 
 
Through Mr. JH's relationship with Health Coach Oluwakemi Adeoso he started to address those fears and ultimately became comfortable with the idea of going to a doctor. As part of the Care Plan, Health Coach Oluwakemi Adeoso worked with the client to understand where he would like to go to be seen by a physician along with the qualities he would like in a physician. Health Coach Oluwakemi Adeoso also gave Mr. JH guidance and support about the importance of managing his own health care. 
 
From the time Mr. JH was enrolled to when he finally went to the doctor, there was a two to three week gap. During that time, Health Coach Oluwakemi Adeoso worked to address any insecurity around health care access and empowered the client to manage his own health care. During the process, Health Coach Oluwakemi Adeoso followed up with the client to ensure all scheduled appointments were kept. Health Coach Oluwakemi Adeoso provided Mr. JH with additional support in order for the client to feel comfortable during the visit. Health Coach Oluwakemi Adeoso also addressed this gap in care by following up regularly leading up to the doctor's appointment in order to make sure it would be attended. As a result of her intervention, Mr. JH is now better managing his diagnosis and regularly taking the medications he needs. 
SIPCW's "Breaking the Stigma" Training

Staten Island Partnership for Community Wellness is offering a 2- hour training that educates health professionals about stigma and bias associated with mental health and substance use.

Participants will learn the difference between stigma and bias, and receive guidance on how to engage with individuals with mental health and/or substance use issues.

PREP: New OASAS Certified 820 Program at Project Hospitality

PREP (Pause, Reflect, Engage, and Prepare) is a new OASAS Certified 820 Program which provides  two elements of care; stabilization and rehabilitation. These services are offered in a person-centered, trauma-informed setting. It's designed for individuals to have a safe place to address their substance use as well as other areas in their life which may be impacted by their use. There will be a compliment of medical and behavioral health services on site including primary care, MAT and Ancillary Withdrawal services, mental health and psychiatry, substance abuse counseling, individual and in a group setting, housing and vocational services, care management, patient navigation services and other holistic services. The site will operate according to the Sanctuary Model, from its new warm and welcoming space, to the care and attention you will get from each staff person upon walking in the door.
 
Value Based Payment Corner

Staten Island PPS launched its Value Based Payment Resource Center earlier this year. This resource is hosted on the SI PPS website and supports our overall initiative to educate and prepare the SI PPS Partner Network for the transition from "fee-for-service" to "value based payments," including the SI PPS VBP Education Program.  

The third in-person session of the SI PPS VBP Education Program - VBP Data Analytics - took place at the SI PPS PAC Meeting on June 26th and covered the keys to success for VBP data analytics. We have added a recording of the presentation given by Jason Ganns, the Director of Bureau of Performance Management and Quality Improvement at the NYS DOH, to our VBP Resource Center, found here, along with the supporting document distributed to attendees. We have also added the link to VBP University from the NYS DOH, which is designed to be an academic resource to raise awareness, knowledge, and expertise in the move to VBP.

 Further content will be posted as our curriculum continues to roll out in 2018.

Patient Activation Training Available on August 28th, 2018

Patient activation refers to how capable an individual is of taking care of themselves when it comes to managing their health and healthcare. Different people have different levels of activation. In this training, participants will learn how to assess a person's level of activation using the Patient Activation Measure (PAM©) and how to tailor goals and interventions for an individual based on their level of activation using Coaching for Activation (CfA©).
