TMF Recognizing
Hospitals for Achievement in Quality
Improvement

 
TMF Health Quality Institute, along with the Texas Hospital Association, Texas Organization of Rural and Community Hospitals, Arkansas Hospital Association, Oklahoma Hospital Association and Oklahoma State University Center for Rural Health, has launched the 2016 - 2017 Hospital Quality Improvement Award program.

Since its inception in 2004, this initiative has recognized hundreds of hospitals that have achieved exemplary outcomes in improving patient care. Award recipients are recognized at formal celebrations and featured in major newspapers and trade publications.

Congratulations to the 2016-2017 Physician Practice Quality Improvement Award Recipients
 
TMF Health Quality Institute, along with partner organizations, is pleased to announce that 97 physician practices are the proud recipients of a Physician Practice Quality Improvement Award. These practices have demonstrated excellence in health care quality through exemplary performance on national quality priorities. View the list of recipients .
 
To receive an award, practices must have met established thresholds in all of the following four categories:
  • Exemplary performance on nationally recognized clinical outcomes measures
  • Regular use of population care management methods
  • Participation in a data reporting quality initiative
  • Implementation of tools and processes to promote patient engagement and improve the patient's experience
Award-winning practices will receive commemorative items to display and will be recognized in statewide media releases. Announcements will also be made in member publications and at meetings by partner organizations: the Texas Medical Association, Texas Osteopathic Medical Association, Arkansas Academy of Family Physicians, Arkansas Osteopathic Medical Association, Oklahoma State Medical Association, Oklahoma Osteopathic Association and the Puerto Rico Medical Association.  Learn more about the award program

TMF Wins Two
Special Innovation Projects
 
The Centers for Medicare & Medicaid Services has awarded TMF two Special Innovation Projects involving Chronic Kidney Disease (CKD) and Chronic Obstructive Pulmonary Disease (COPD).

COPD Project  
The COPD project's aim is to provide chronic disease management focusing on rapid recognition and proper self-management of COPD exacerbation. TMF is developing a "COPD rescue pack" for use by patients when experiencing a COPD exacerbation. The rescue pack includes an instructional packet on COPD, a tool to help patients identify symptoms that may lead to an exacerbation, a prescription protocol, self-care information and a contact card for direct access to a clinician.
 
This project is expected to reduce COPD emergency department utilization and subsequent inpatient admissions while empowering patients to manage their condition. It is identified as a Local Quality Improvement Intervention that involves 60 primary care providers across Arkansas, Missouri and Oklahoma.
 
CKD Project
The CKD project is building on the success of TMF's Health for Life - Everyone with Diabetes Counts project. It promotes and increases timely screening, diagnosis and treatment of CKD in primary care practices. Additionally, TMF is working with diabetes self-management education (DSME) trainers and patients to teach and empower patients to seek and obtain CKD screening.
 
The project includes two primary interventions:
  1. Implementing a comprehensive provider approach focused on improving awareness of clinical guidelines, incorporating an EHR-enabled workflow and implementing office-based patient engagement strategies
  2. Expanding the scope of TMF's current DSME efforts by augmenting patient understanding and engagement in CKD care
The CKD project is engaging 125 providers in rural areas across Arkansas, Missouri, Oklahoma, Puerto Rico and Texas, with a focus on disparate populations and impacting 5,000 at-risk patients and their families. A key teaming partner on this project is the National Kidney Foundation, which is providing educational events and patient education materials.
 
More than 30 million people or 15 percent of US adults are estimated to have CKD. About 48 percent of those with severely reduced kidney function, but not on dialysis, are not aware of having CKD; and, most (96 percent) people with kidney damage or mildly reduced kidney function also are not aware of having CKD.
