NYS DOH DSRIP ANNOUNCEMENTS

RFA FOR $195 MILLION IN CAPITAL FOCUSED FUNDING
The Department of Health (DOH) issued a Statewide Health Care Facility Transformation Program (SHCFTP) Request for Applications (RFA) for $195 million in new funding dedicated to healthcare providers for capital and debt restructuring purposes, click here to read more. Applications must be submitted by September 16.
  
DOH will hold an informational webinar in early August on a date to be determined. The SCC will announce the webinar date once announced. Immediate q uestions may be emailed to healthcarefacilitytransformation@health.ny.gov. Questions and answers will be posted on or about August 17, 2016.
  
CBO PLANNING GRANT RFA DUE DATE EXTENDED
The RFA Application Due Date has been extended to August 18, 2016. This Addendum can be viewed here
 
Please note that organizations must be prequalified in the Grants Gateway to submit an application for this opportunity. The current Grants Gateway prequalification expiration is August 15 and this RFA application extension recognizes that to ensure prequalification activities should be met or renewed first prior to the Grants Gateway RFA submission.

CULTURAL COMPETENCY & HEALTH LITERACY 101
Special thanks to members of the CC & HL Workgroup for your contributions to the new curricula!
 
The Cultural Competency and Health Literacy (CCHL) Workgroup has completed the first in a series training presentations, entitled CCHL 101.  A subgroup of the workgroup diligently worked to produce an informative educational introduction to cultural competency and health literacy for leadership, providers, and organization partners across the PPS as well as the SCC team.   
 
CCHL 101 will be hosted on the SCC website in the Learning Center, in the meantime please click here to access the module.  There is a quick registration process and 5 brief multiple-choice post questions for each module.  We encourage all our partners to log-in and preview this valuable training tool.  Look for more information about this training and how your organization might collect data for training reports in the next edition of Synergy.
SUMMER FOOD SERVICE PROGRAM!
Join ISLAND HARVEST this summer for the summer food service program!
 
The Summer Food Service Program, sponsored by the U.S. Department of Agriculture, provides free, nutritious meals and snacks to help children in low-income areas get the nutrition they need to learn, play, and grow, during the summer months when they are out of school. Families are invited to visit Summer Food Service Program sites during the summer months to receive a meal.  Island Harvest partners with over 65 organizations over the summer to feed children. Any child or teen under 18 can visit to get a free lunch! More information about this program can be found here.
 
SMOKE FREE CAMPUS INITIATIVES AT BEHAVIORAL HEALTH SITES
  
In 2015, the National Council for Behavioral Health launched the Wellness and Recovery Learning Community an eight-month initiative comprised of seven Florida-based addictions provider organizations and programs to support providers in developing and implementing evidence-based tobacco cessation strategies for people with addictions. Eight months later, all have gone tobacco free or have processes in place to help their clients lead a tobacco free life.
 
Three organizations from this learning community have offered to share their experiences.  Join us on Monday, July 25th from 2:00-3:30 pm EST for the Building Partnerships to Reduce Tobacco Use Among People with Addictions webinar and hear your peers reflect on their experiences going tobacco-free, best practices and lessons learned in integrating tobacco cessation into addictions settings and progress to date. INTEGRATED SERVICE SETTINGS OPEN DOORS FOR YOUTH EXPERIENCING HOMELESSNESS

Young people through the age of 24 make up about one-third of all persons experiencing homelessness and the U.S. government aims to end youth homelessness by 2020. Providers in integrated care settings can play an important role in meeting this challenge as seeking health care can be a young person's first step to leaving the street. Youth experiencing homelessness have a complex array of medical, behavioral and social problems that benefit from the skills of staff working in integrated care sites.
 
Visits for clinical care offer providers a window of opportunity to address the immediate needs of youth experiencing homelessness and to work on long term goals in settings youth consider safe and trustworthy. Join this webinar to enhance your knowledge and skills for working with youth experiencing homelessness and with the social service agencies key to securing housing and other supports.

WELCOMING NEW STAFF TO THE SCC!
  
Ralph Thomas, MHA, Project Manager, Care Transitions
As the new Project Manager, Care Transitions, Ralph will be managing the INTERACT program in addition to the PMO's post-acute care program initiatives. Ralph will also be supporting the RHIO connectivity requirements and developments of our Skilled Nursing Facility partners.
 
Teamwork Team Together Collaboration Meeting Office Brainstorming Concept Ralph is a graduate of the Hofstra Masters of Health Administration program and received his undergraduate degree in Health Science with a concentration in public and community health education from Stony Brook University. Most recently, Ralph has completed an administrative internship at Northwell Health IT Project Management Office of the CIO. During his PM experience he worked collaboratively with cross-functional teams and technical staff to manage projects. Ralph's experience also includes work at the Montefiore School of Health Programs as a Community Health Organizer where he executed and developed the implementation of collaborative health promotion initiatives on a classroom, university and community-level. This included conducting over 100 health education workshops and youth development groups. Further, Ralph contributed to bridging the gap between community health needs and research by working as a Project Aide at the Community Alliance for Research Empowering Social Change (CARES).
