The Department of Health Care Policy and Financing (Department) is working with our partners to develop the next phase of the Accountable Care Collaborative (ACC). Phase II of the ACC seeks to leverage the proven successes of Colorado Medicaid's programs to enhance the Medicaid client and provider experience. Learn more about ACC Phase IIThe goal of ACC Phase II it to improve health and control costs by:
  • Fostering the integration of primary care and behavioral health services,
  • Improving coordination of services by advancing team-based care, Health Teams, and Health Neighborhoods,
  • Promoting client choice and engagement,

Update of Request for Proposals
Currently, the ACC Phase II Team is developing a working draft of the request for proposals (RFP) that includes the major pieces of the program.

The  Department's  ACC Program Improvement Advisory Committee (PIAC)  and other stakeholder groups have also be asked for input on major program decisions.  We encourage all interested parties to see the  past meeting summaries on the ACC Phase II site  that captures discussions around specific Phase II topics.

In fall 2016, the stakeholder community will have a chance to weigh in on the draft RFP. Between now and the release of the draft RFP, the Department will not regularly solicit stakeholder feedback.

Joint Budget Committee Hearing
Earlier this month, the Department provided an update to the Joint Budget Committee (JBC) on ACC Phase II. At the hearing, Medicaid Director Gretchen Hammer, Office of Community Living Director Jed Ziegenhagen and Deputy Medicaid Director Laurel Karabatsos addressed Committee questions regarding the Department's budgetary and legislative authority for the program and how individuals with disabilities are served by the ACC.

The JBC was also provided the following resources available on the ACC Phase II site:
