Top 5 Measurements of PHM Program Effectiveness This Week's Challenge: Clinical outcomes is the metric most often evaluated to measure and improve population health management (PHM) program effectiveness, say 78 percent of healthcare companies who responded to HIN's first annual survey on Population Health Management. We wanted to see which other metrics are reviewed to monitor a program's effectiveness.

What We Learned: HIN's Population Health Management survey, conducted in September 2012, documented the efforts of 102 healthcare organizations in population health management, including program areas, populations served, how the appropriate level of intervention is identified and much more. According to the companies who responded to the survey, the top five metrics evaluated to measure and improve PHM program effectiveness are:

  • Clinical outcomes: 78.4 percent

  • Patient satisfaction: 75.7 percent

  • Patient compliance: 54.1 percent

  • Health claims: 48.6 percent

  • Rx claims: 40.5 percent
