This Week's Challenge: Case managers are the health professionals found most frequently on a population health management (PHM) team, say nearly 64 percent of healthcare companies who responded to HIN's first annual survey on Population Health Management. We wanted to see which other health professionals are part of the PHM team.

What We Learned: HIN's Population Health Management survey, conducted in September 2012, documented the efforts of 102 healthcare organizations in population health management, including program areas, populations served, how the appropriate level of intervention is identified and much more. According to the companies who responded to the survey, the top five health professionals on the PHM team are:
- Case manager: 63.8 percent
- Primary care physician: 59.6 percent
- Health coach: 55.3 percent
- Nurse practitioner: 40.4 percent
- Specialist: 36.2 percent
