The National Committee for Quality Assurance (NCQA) is offering a free webinar series this fall on the Patient-Centered Medical Home (PCMH) titled A Toolbox for Transformation to the PCMH Webinar Series.

Webinar 1 - Building the Foundation: The Joint Principles of PCMH (10/23/13 from 1:00pm-2:30pm EST):
A review of the Joint Principles of the PCMH, supported by four national primary care physician organizations, and key characteristics of the PCMH and how they align with NCQA PCMH requirements and efficacy of evidence-based guidelines.

Webinar 2 - Lessons from PCMH Champions (11/7/2013 from 2:00pm-3:30pm EST):
Redesigning care management coordination is the key to practice transformation. In a PCMH, job descriptions and roles are ever evolving to incorporate the concept of team-based care. This webinar will focus on organizational values and restructuring a primary care team to achieve optimal care.

Webinar 3 - Quality Resources for Developing Your PCMH Project Plan (11/20/2013 from 2:00pm-3:30pm EST):
Primary care providers are challenged by implementing a PCMH and completing their normal tasks while undergoing practice transformation. This webinar will highlight resources to assist with practice transformation barriers and facilitators to PCMH implementation.

This webinar series is designed for any health care provider, decision maker, or quality improvement consultant interested in the PCMH transformation process.
