The patient centered medical home: mental models and practice culture driving the transformation process

A recent study conducted by the PA SPREAD team found providers and staff must change their thinking to successfully implement the PCMH. Practices must:

1. Shift their perspective towards proactive, population-oriented care based in practice-patient partnerships.

2. Create a culture of self-examination.

3. Distribute responsibilities and develop new roles within the practice in adopting team-based care.
