Some believe that an ACO’s leadership structure predicts its success. They point to differing savings results for physician-led versus hospital-led ACO shared savings models (MSSPs) to make their case. In particular, they make the argument that future Value-Based Care (VBC) policies should benefit the growth of successful physician-led ACOs, protecting them from policies that force them into Risk. There are significant flaws to tying ACO structure to the viability or value of the model, however, or in discrediting ACOs which don’t yet produce those savings but may have other important characteristics for the long term.

It has been clear for years that social determinants of health (SDOH) – factors such as education, economic and housing stability, community support, and access to transportation – have a disproportionate impact on health outcomes. Research shows that SDOH accounts for up to 80% of health outcomes, while medical care – which costs the U.S. nearly $4 trillion annually – accounts for the balance. What has been far less clear is how providers and payers can fully harness SDOH data to proactively manage the health of patients and members. 
The Medicare Shared Savings Program (MSSP) is the largest value based care program from the Center for Medicare & Medicaid Services (CMS) to date. Since its inception in 2012, Accountable Care Organization (ACO) participation has doubled; providing care to over 10 million beneficiaries, generating $1.94 billion in total savings, and earning over $1.47 billion in shared savings in the latest 2019 performance year (PY). The purpose of this program is for ACOs to coordinate the following....
For the first time, most physicians worked outside of physician-owned practices in 2020, as doctors continue to gravitate toward employment by hospitals and other organizations, according to a new American Medical Association survey. The trade group’s latest Physician Practice Benchmark Survey found that 49.1% of patient care physicians worked in physician-owned practices in 2020, a drop of almost 5 percentage points from 2018, when that figure was 54%. It’s 11 percentage points lower than 2012, when 60% of physicians worked in physician-owned practices.
