The Depression CAREPATH was developed for use in home health care and hospice to support the assessment, monitoring, and case coordination of depression in older adults. Patient education materials, communication aids, and implementation guides and resources for administrators are also available.
After you complete registration for HWapps, the link found in the content section will direct you to a registration form for mental health training network, where you will access training.

Asthma Educator Institute (AEI)
Enroll in a two-day American Lung Association preparatory course for the National Asthma Educator Certification Board (NAECB).
This free Asthma Educator Institute (AEI) training course, a $250 value for each participant, is funded by Healthfirst. Led by NY metro-area practitioners who are experts in the content areas, the course provides training in the National Heart, Lung, and Blood Institute (NHLBI) and National Asthma Education and Prevention Program (NAEPP); guideline-directed asthma care to patients, families, and other healthcare providers.
The curriculum covers the content outlined in the NAECB Candidate Handbook and includes case reviews, hands-on skills demonstration, and practice. In addition, the two days provide a unique opportunity to network with a select group of healthcare providers, educators, and organizations involved in asthma care and asthma care policies.
Requirements:
1.
Please forward the registration form to the practices that you recommend so that they may reserve a space in the program.
2.
Since the goal is for the participants to take and past the actual Certification test, we ask that the practices demonstrate that their staff are indeed registered to take the test soon after the completion of the course. The registration for Certification can be found at

. Staten Island PPS will consider covering the $295 fee.

This bootcamp will prepare students to pass the LMSW/LCSW exam.
Bootcamp includes:
- A thorough review of the national association of social work code of ethics with questions to ascertain mastery of the material.
- Detailed review of DSM Axis I & Axis II, Defense Mechanisms, Family & Couples Treatment, Community Organization, & Group Therapy. All sections will be reinforced with questions for review.
- Review of Administration, Supervision, Diversity/Cultural Competence, Addictions, Communication Techniques, Theoretical Approaches, Theories, and Psychotropic Medications.
- Participant will be given a workbook (covering all content needed to pass exams) AND complete audio CD set (8 CD's) that reinforces the workbook.

Patient activation refers to how capable an individual is of taking care of themselves when it comes to managing their health and healthcare. Different people have different levels of activation. In this training, participants will learn how to assess a person's level of activation using the Patient Activation Measure (PAM©) and how to tailor goals and interventions for an individual based on their level of activation using Coaching for Activation (CfA©).

Care Coordination Basic and Advanced |
Care Coordination Basic and Advanced
The Care Coordination Fundamentals curriculum consists of four (4) modules or sections that are structured to build on one another sequentially. All front-line staff working in a team-based healthcare environment will benefit from this course. This advanced version of the care coordination curriculum introduces staff to chronic, mental and behavioral conditions, (i.e. diabetes, hypertension, high cholesterol, asthma, depression, substance abuse etc.) and helps them understand these conditions and provides participants with coaching tools on how to teach patients to manage their health.
Participants will experience a highly interactive class environment tailored to adult learners. Our approach strengthens the participants' critical thinking skills by engaging them in discussion, individual exercises and group activities. Participants complete the course prepared to assist their patients in navigating the healthcare system, and to be strong, productive members of healthcare teams providing coordinated and patient-centered care.

Disability Ally Training
Person Centered Care Services of Staten Island is striving to develop relationships with agencies, business', hospitals and all community members to provide training/education on interacting/working with people with disabilities. They have experienced that after people are given the education of disabilities culture, communication tips, and general etiquette better relationships are possible. This training can be geared and modified for anyone in any role or capacity. For example, if they are training hospital staff, the training can be geared to focus health literacy and communication/comprehension of medical issues/treatment.

LGBTQ Cultural Awareness
LGBTQ Cultural Awareness Training, presented by the Pride Center of Staten Island, advances a person-centered approach to working with LGBTQ patients and/or clients in healthcare settings. The training includes an introduction to (or review of) LGBTQ terminology, activities that focus on intersectionality and structural systems of oppression, and a discussion of the health disparities confronting LGBTQ individuals. Additionally, the training will give you concrete tools to make healthcare environments more welcoming for LGBTQ patients and/or clients, as well as your LGBTQ co-workers.

Web-Based Disease Management/Care Management Training
xGLearn combines the latest collaborative technology with distance learning to enable care management team members, including RNs, LPNs, Social Workers, Physicians, Physician's Assistants, Nurse Practitioners, Community Health Workers, Behavioral Health Workers, and Pharmacists to function at the top of their licenses. Through xGLearn, care management teams obtain the evidence-based knowledge required to assess patients, develop action plans, reliably implement evidence-based practices, and coordinate care.
