North Jersey Health Collaborative Receives National and Local Recognition as a Recipient of two Grant Awards:
|
North Jersey Health Collaborative is one of 19 communities selected to participate in the nationwide BUILD Health Challenge, a unique national program focusing on bold, upstream, integrated, local, and data-driven projects that can improve community health.The BUILD Health Challenge grant provides funding ($250,000 over two years), capacity building support, and access to a national peer learning network to enhance collaborative partnerships to locally address our communities most pressing health challenges.

North Jersey Health Collaborative has been awarded a $150,000 grant, over two years, by the Horizon Foundation of New Jersey, a philanthropic arm of
Horizon Blue Cross Blue Shield of New Jersey
. This grant will fund a Diabetes Intervention Pilot Program in Morris County. In partnership with Zufall Health, the pilot program will deploy a Community Health Worker to help identify adults living in Morris County who are pre-diabetic or diabetic and enroll them in a six-month pilot program. The Community Health Worker will follow enrolled individuals and coordinate connection to a range of services as appropriate, including referral to a primary care provider, community resources, social services, education, and peer support. This pilot program is aligned with the strategies of the Morris County Diabetes/Cardiovascular Disease Workgroup. Stay tuned for more information and details on this exciting pilot program!

workgroups.
Workgroups will give a verbal report on their performance measures to help track our collective progress.

NJHC Workgroup Spotlight
Union County: Diabetes, Hypertension & Health Literacy
The Diabetes, Hypertension & Health Literacy workgroup held a community health fair in Vauxhall, bringng blood pressure screenings, glucose testing, information and resources to the community. Overlook Medical Center provided food and music adding some festivity to the event. Sheri Cognetti, who leads the workgroup shared, "except for the unexpected rain, things went really well. We had the opportunity to engage with community members, provide an important service, and even enlisted a few new partners in our workgroup." Plans are underway to bring the health fair to other communities in need.

This video was made possible as part of the American Association of American Colleges (AAMC) 2017 Health Equity Research Snapshot. The NJHC video is one of seven videos representing exemplary community-partnered CHNA processes from AAMC member hospitals across the country, including Atlantic Health System,
that have successfully impacted the health of local residents.
The AAMC solicited videos from member hospitals to demonstrate the diverse efforts that aim to understand and eliminate health inequities. For more information about this project go to the AAMC website
