Celebrate National Health Center Week Across the Country

National Health Center Week will take place on August 13th - 19th. Across the US, Community Health Centers will hold events throughout this week focusing on topics such as health screenings, public housing, agricultural worker health and children's health. For a full list of Health Center Week themes, click  here. Health Centers are encouraged to hold health fairs and events during this week to support their community. Events do not have to coincide with the daily themes. If your health center has a public event or is looking to visit other events, visit the events page . Search by day, state or zip code.

East Coast Migrant Stream Forum Registration Information Available
 
 
Registration for the East Coast Migrant Stream Forum is now open. The conference will take place on October 12-14th in Atlanta Georgia at the Renaissance Atlanta Waverly Hotel.

The East Coast Migrant Forum is an annual conference offering training and development opportunities for professionals in the agricultural worker health field. Session topics include Outreach, Policy and Research. For a draft 2017 conference program,  click here. Please check back closer to the conference date for a final program.

Scholarships are available for outreach workers and community health workers. All outreach workers are encouraged to apply. Registration can be done online.

Some deadlines to keep in mind:

Scholarship Application Deadline: August 25th
Early Bird Registration Deadline: September 8th
Hotel Booking for Group Rate: September 19th at 5pm ET
Online Registration Deadline: October 6th

Using 'Choose MyPlate' Tool for Healthy Eating


MyPlate is a bilingual online tool for building healthy eating habits for people of all ages. It creates an easy to follow guide, taking into consideration dietary needs, age and other factors, to help create a healthy diet. 

In addition to their online calorie and nutrition tracker, they offer information about how to read food labels, healthy recipes and portion sizes. 

Part of a healthy diet includes being active and fit. MyPlate also has information about physical activities and even gives estimates on how many calories certain activities burn. Tips provided are for all sorts of life styles and ages. Some suggestions include walking during a lunch break at work, walking the dog, pushing a baby in a stroller or playing with your child. 

MyPlate has tools for children as well, including growth charts and calorie level (with portion recommendations) for different age groups. These tools can be printed and tracked.

Health Centers can also utilize these resources, which are all offered for free. Dozens of infographics are available on various topics from being active, to portion size, and even to healthy recipe cards. PRAPARE: A New Protocol on Social Determinants of Health for Health Centers

PRAPARE stands for the Protocol to Respond to and Assess Patient Assets, Risks and Experience. It is a tool created by the National Association of Community Health Centers (NACHC) to address Social Determinants of Health (SDOH). 

Addressing SDOH can aid health centers to develop non-clinical methods such as enabling services to address causes of patients' health concerns which can reduce the total cost of care while improving patients' long-term outcomes.

The PRAPARE tool is a series of questions designed to identify SDOH. Some of the questions include:
"Are you worried about losing your housing?"
"What is your current work situation?"
"Has lack of transportation kept you from medical appointments, meetings, work or from getting things needed for daily living?"
NACHC has created a working document that can be found  here.

NACHC has also created the PRAPARE Implementation and Action Toolkit to help interested users to get started. This contains step by step guides to help understand the PRAPARE project, answer commonly asked questions and evaluate collected data.
