When first signing into MyChart you may notice your child's immunizations are flagged as overdue...
The CDC recommends Varicella, MMR and DTaP and Polio vaccination between 4-5 years old. Our practice, as most, choose to give these vaccinations at the 5 year visit - prior to entrance to kindergarten. The new system flags these vaccines are overdue once a child turns 4 years old. Do not worry, if your child has not yet had their 5 year well visit they are not overdue!

has switched to a new electronic medical records system that we believe will very much improve your child's experience at our office. While we adjust to this new system, things like checking in and out at the office and visiting with your child's provider may take longer, and some of our office procedures may be a little different the next time you visit.
We apologize for the inconvenience.
One great advantage of our new system is MyChart, which enables you to view your child's medical record, including notes from medical visits and lab tests, get past immunization history, view and pay your bill, and even to download camp and sports forms.
On your phone, download the MyChart mobile app from your application center. Open the MyChart app, select your country and state, and pick "PPOC" (see logo bottom right) from the listed organizations. Then, log in!

If you have any difficulties you can obtain an activation code at your next visit, or over the telephone. It requires additional information such as (your) date of birth and contact information.
