Around HCCI we talk a lot about transformation. How great home-based primary care transforms the lives of patients and family members. How this wonderful care transforms the people who provide it, and how this unique model has the potential to transform our health care system.

You're a part of that transformation! You're at the forefront of a movement that will change the way we care for our most vulnerable patients, improving patient outcomes and experience, in a fiscally sustainable manner. Together, we just might save Medicare!

On that note, I'd like to share with you a collection of moving quotes about why home-based primary care is so important to everyone involved. See it here, in our  Home-Based Primary Care Quote Gallery

Until next time, please visit us often at hccinstitute.org to see what's new!

Why HBPC Training Matters

Health care professionals are already accustomed to following established practice guidelines and strict treatment protocols. However, until now, no such road map has existed for practitioners caring for chronically ill and other medically complex patients in the home environment. HCCI's program offers expert guidance for applying a consistent, reliable, evidence-based approach to the unique aspects of practicing in this non-traditional environment.

HCCI Essential Elements of Home-Based Primary Care is a 3-part sequential learning experience designed to offer a clear path to professionals looking to sharpen their clinical and practice management skills.

Part one is being offered at eight HCCI Centers of Excellence
across the US, and registration has begun. Visit  us at HCCInstitute.org/train for training dates and locations, instructor information, or to enroll.

HCCI University Coming Soon!

In addition to launching our first instructor led course, HCCI Essential Elements of Home-Based Primary Care ,  HCCI is on the cusp of unveiling a new learning management system for online education. Each series of courses will allow learners to explore various aspects of HBPC in more detail. Expected to kick off in late October and grow over time, HCCI University is intended to take learners past the foundational level and deeper into clinical and practice management competencies. Check back often for new offerings.

HCCI's educational offerings are made possible through the generous support of The John A Hartford Foundation and the Bramsen Foundation
