That long-delayed admission came one chilly Christmas Day when the presents I put under the tree received (yet again) an even chillier reception from my family members. DUFFY HEALTH CENTER AWARDED
$15,000 STRATEGIC FOCUS GRANT
FOR ENHANCED TECHNOLOGY
Duffy Health Center received one of five Strategic Focus Grants from The Cape Cod Foundation this year. The center, which provides primary care and behavioral health services for those who are homeless or at risk of homelessness, received $15,000 to support implementation of a web-based health technology tool called DRVS. The Massachusetts Department of Public Health now requires health care practices to have an online tool that offers real-time data and reports to improve patient outcomes and to drive and measure improvement in population health management. The DRVS platform will help Duffy enhance patient care, provide the documentation needed to be recognized as a quality cost-effective provider, and meet the long-term technology demands tied to the Accountable Care Act.

COMMUNITY HOUSING PARTNERSHIP AWARDED $20,000 FOR COMMUNITY EDUCATION

The Cape Cod Foundation has awarded $20,000 to the Community Housing Partnership. The initiative, spearheaded by the Community Development Partnership and Housing Assistance Corporation, is designed to build community support for affordable housing.  The year-long project, which begins this fall, revolves around educating members of the Cape community about affordable housing development and management to, in turn, receive the support necessary to effect change. READ MORE
