The ABCs of Anticoagulation and Reversal:
Do You Know Your DOACs?

Designed and delivered by AC Forum experts Steven B. Deitelzweig, MD, MMM, FACC, SFHM, FACP, RVT, David Garcia, MD, and Scott Kaatz, DO, this free, online program aims to provide comprehensive updates and strategies for the reversal of oral anticoagulants in order to deliver the best possible care.

At the conclusion of this activity, participants will be able to:
  • Recognize which patients are candidates for reversal and the potential thromboembolic consequences of discontinuing and reversing anticoagulation.
  • Discuss the evidence to reverse warfarin with Vitamin K, FFP and PCC.
  • Examine the role and clinical utility of reversal agents for various DOACs.
  • Explore other reversal agents that are currently in development.

Compendium of Resources

The AC Forum is pleased to announce that our Centers of Excellence program has been included in a new Compendium of Resources developed by The Joint Commission's Department of Health Services Research. This compendium promotes reliable and up-to-date patient education and discharge materials for VTE and anticoagulation therapy.

The Joint Commission embarked on an 18-month research project, with funding from Bristol Myers-Squibb / Pfizer, Inc., to determine what, if any, patient education materials are being provided to patients discharged on anticoagulants other than warfarin. As a result of this work, a compendium of resources was compiled with links to publicly available resources to promote the provision of reliable and current patient education and discharge materials for VTE and anticoagulation therapy.

Centers of Excellence New Resource Center

The AC Forum's Centers of Excellence has launched a new resource library with a robust search tool and dynamic interface for use on a variety of devices. The 350 resources on the site are now organized with intuitive headings and subheads, making the information scannable and more accessible. The resource center also highlights current practices that have achieved recognition as a Center of Excellence. Visit the new resource at the Centers of Excellence website.

Created in 2012, the AC Forum's Anticoagulation Centers of Excellence program helps healthcare professionals provide the highest level of care to their patients on antithrombotic medication. In addition to the resource center, this free program offers a practice self-assessment with specific guidelines and recommendations for best practice.
