The New Jersey Division of Developmental Disabilities (DDD) serves individuals 21 years of age and older. Please read the important information below sent out by DDD on June 23rd, 2017

Please accept our apologies and note the following correction (highlighted) to the Division Update sent earlier this morning:
 
 
The Division continues to urge individuals who currently participate in the DDD Self-Directed Employee Option to initiate enrollment as the employer of record with Public Partnerships TODAY, Friday June 23.
 
For individuals who already have begun the registration process , we have extended the deadline until PPL has fully enrolled your self-directed employees so that there is no disruption in service.
 
For individuals who have not yet started the enrollment process , starting Monday, June 26th, PPL will be outreaching every family to  expedite and facilitate the application. A PPL single point of contact will be established for each family.
 
Remember, the only exception   to the need for immediate action is the 90-day extension granted to individuals dually enrolled in both PPP (Personal Preference Program, administered by the Division of Disability Services) and the DDD Self-Directed Employee Option.  Please note that the 90-day extension applies only to the DDD services of dually enrolled participants - it does not apply to the PPP services.

Public Partnerships Enrollment for Individuals/Families Currently Participating in the DDD Self-Directed Employee (SDE) Option
  • Online Enrollment (fastest)
  • Phone Enrollment: 844.842.5891
  • Information Needed: individual's name, last four digits of individual's social security number, valid email address for the individual
 
Please note :
  • Easter Seals will make SDE payments for all DDD-funded self-directed services rendered through June 30. SDE timesheets for services rendered through June 30 should be submitted to Easter Seals.
  • Individuals who only receive DDD-funded self-directed services from a Qualified Provider Agency (and not from a 'self-hire') do not need to complete online or phone enrollment. In this case, only the Qualified Provider Agency needs to enroll, as service recipients' information has already been transmitted to Public Partnerships.
  • Participants experiencing issues that are delaying their enrollment should contact the Division's Fee-for-Service Help Desk: [email protected].us. Please include the individual's six-digit DDD ID # and be as detailed as possible about the issue/s.
 
Public Partnerships Enrollment for Qualified Provider Agencies
  • Agency enrollment consists of submitting the Agency W-9 Form (required) and the Agency Payment Information Form (optional) to Public Partnerships via Fax or Email. Forms are available at Public Partnerships-DDD, under the Qualified Providers tab.
  • Easter Seals will make Qualified Provider Agency payments for all DDD-funded services rendered through June 30Vouchers for services rendered through June 30 should be submitted to Easter Seals.
  • Qualified Provider Agencies that have not yet enrolled should do so immediately to ensure billing through Public Partnerships can begin July 1.
