The next iteration of the Accountable Care Collaborative (ACC) is scheduled to begin July 1, 2018. The goals of the next phase are to improve member health and reduce costs.

In November 2017, the Department announced the awards for the seven Regional Accountable Entity (RAE) contracts. The Department has made available the proposals from each of the awarded entities on CO.gov/HCPF/ACCPhase2

Attribution Webinar-Register Now
ACC Phase II Attribution Webinar
Date: February 28, 2018
Time: 3:00-4:00pm
Purpose:
  • Review the process for attributing members to a provider under Phase II
  • Share what provider actions are needed
Participants will be provided a Q&A opportunity through the webinar chat function.

 
Participation is capped at 200. The webinar will be recorded and posted on CO.gov/HCPF/ACCPhase2

ACC Phase II Implementation Process Overview
On January 30, 2018, the Department hosted a webinar for providers, stakeholders and other interested parties that delivered a high-level overview of the implementation process for ACC Phase II. The purpose of the webinar was to:
  • Review goals and objectives of ACC Phase II
  • Share Department implementation activities and key milestones
  • Share Plan for external communications
 
ACC Phase II: Provider Contracting (What Providers Need to Know)
On February 7, 2018, the Department hosted a webinar for providers that offered information about contracting with the Regional Accountable Entities. The purpose of the webinar was to:
  • Review goals and objectives of ACC Phase II
  • Share role of Regional Accountable Entities (RAEs)
  • Share what providers need to know and action needed

Timeline of Activities & Resources
The Department continues to prepare for the implementation of the next phase of the ACC and is working hard to ensure a smooth transition of the program. Along with immediately focusing on the RAE contract negotiations, finalizing the 1915(b) waiver with the Centers for Medicare & Medicaid Services (CMS), managing system updates and other activities, the Department is preparing and training staff for the changes in operations. As a result of all of the mission critical activities underway, the Department is limited in its ability to respond to individual questions and request for presentations. In an effort to be responsive to provider, member and stakeholder needs, the Department will continue to develop key communications listed below about the program over the next few months. As new information and resources become available we will announce it in this newsletter and post them on CO.gov/HCPF/ACCPhase2 .

Additionally, interested parties can submit their questions to the Department using this form on  CO.gov/HCPF/ACCPhase2 . The Department will not be able to respond directly to all inquiries but will use the questions submitted to tailor upcoming communications and resources.
