30 Day Readmission Alert Notification Service
HEALTHeLINK is assisting participating providers in proactively managing their hospital readmission rates. For each discharge ADT generated by WNY's hospitals, HEALTHeLINK will auto-generate, in real-time, a subscription on the discharged patient. Each subscription will auto expire 30 days after the discharge date. During the 30 day period, HEALTHeLINK generates real-time alert notifications if your patient is admitted into any WNY hospital. The benefits of this new service include:
  • Receive real-time alert notifications when your patients have been re-admitted to any WNY hospital
  • From within the alert message, one-click access to the patient's full HEALTHeLINK record
  • Ability to follow up with patients that frequently go to various hospitals
  • Ability to provide information to clinical and discharge planning teams
If interested, contact the HEALTHeLINK Account Management Team at 716-206-0993 ext. 105 or [email protected] to request the Patient Alert Notification Service Agreement to get set up.

HEALTHeLINK Functionality: Did you know?
The HEALTHeLINK standard level 1 consent form allows for access to patient data in very specific use cases:
  1. Treatment means the provision, coordination, or management of health care and related services among health care providers or by a single health care provider, and may include providers sharing information with a third party. Consultation between providers regarding a patient, and the referral of a patient from one provider to another, is also included with the definition of "treatment."
  2. Care Management and Care Coordination Activities include assisting you in obtaining appropriate medical care, improving the quality of health care services provided to you, coordinating the provision of multiple health care services provided to you, or supporting you in following a plan of medical care.
  3. Quality Improvement Activities includes evaluating and improving the quality of medical care provided to all patients.
There are  other uses cases that exist (i.e. research, access to family member data for genetic purposes) that are not permissible based on the standard level 1 consent form.  There may be other consent processes specific to these use cases. Please contact HEALTHeLINK for more information.
