How do Post-Acute Care Providers Use HIN?

"HeathInfoNet is an invaluable service and I always learn more about my patients. I work at multiple Genesis Healthcare locations and its now part of my workflow to keep HIN open along with my electronic health record. It is especially useful where I work at Marshwood Center, since Central Maine Medical Center and St. Mary's provide data to HealthInfoNet.  And now that Southern Maine Healthcare is connected to HealthInfoNet, I have much more information about my patients at Riverwood Center.

When patients return from hospitalization, I can access the discharge summary, more robust records, and test results - which I don't have to repeat. Having consultation information is useful especially for palliative care in geriatric consultations. There are also aspects of a patient's H&P that can be difficult to retrieve upon arrival at a post-acute care facility - such as possible advancing dementia or resolving delirium - and I can often find prior H&Ps or discharge summaries on HealthInfoNet."
