Data Nerds Unite at the All In National Meeting

The All In National Meeting brought together 250+ stakeholders from community collaborations across the nation that are driving population health improvement through multi-sector data sharing. Get the higlights in a Storify recap, summarized by All In staff and attendees on Twitter, and read some of the key takeaways in blogs from AcademyHealth and BUILD. 


Beyond the Next Grant: Sustaining Data Sharing Collaborations

To explore innovative sustainability strategies, All In hosted a webinar and a subsequent plenary session at the national meeting, where experts discussed how they are working to generate reliable sources of ongoing revenue and build capacity to support community health transformation. This blog shares some of their advice for local data sharing collaborations.

Project Spotlights

This month, we're featuring two projects that are tracking performance metrics to improve population health. 

CHP: Essential Access Health

Essential Access Health champions and promotes quality sexual and reproductive health care for all. As a part of the CHP Program, Essential Access Health is a subject matter expert community contributing their expertise in data aggregation, performance measurement and data dissemination. They recently released a learning guide to support population health performance management for those using electronic health data. 


BUILD: The Healthy Ontario Initiative

The Healthy Ontario Initiative seeks to lower the rate at which adults are diagnosed as overweight or obese. To reduce barriers to health, the initiative developed a data platform that defines Key Performance Indicators, establishes baseline metrics, and tracks progress over time. It then created health hubs, including two city parks with community centers, two family health centers, a community garden, four places of worship, and an elementary school.

This blog, featuring DASH grantee HealthInfoNet, profiles how health information exchanges (HIEs) are using data to support population health efforts. HealthInfoNet, Maine's HIE, has taken the lead on bridging the gap between healthcare services and social services.


The Logic of Funding an Ambitious Health Data Project

Improving public health data is a growing point of interest for both domestic and global health funders. A recent Inside Philanthropy article noted that data is the key to BUILD's efforts as well as the increase in BUILD's funders.

  • SIREN Innovation Grants: Due June 15 - This program will support research projects that add to our understanding of the health care cost and utilization impacts of interventions initiated in the context of health care delivery that target patients' social and economic hardships.
  • AHRQ Implementation & Evaluation of Health IT Strategies: Due September 25Apply to stimulate innovative and collaborative research by utilizing new health IT strategies for collecting and using patient-reported outcome measures in primary care and other ambulatory care settings.

  • NNPHI Annual Conference: May 17 - 19 in New Orleans, LA - Join a breakfast roundtable to discuss lessons learned from the All In National Meeting and ways All In can support multi-sector data integration moving forward. 
  • Practical Playbook National Meeting: May 31 - June 2 in Washington, DC -  In this session, All In leaders introduce an instrument for assessing a community's readiness to collaborate and share data. Representatives from current CHP, DASH and BUILD projects will discuss their own evolution within this assessment framework, and how they have used the process and data to inform next steps.
