Advance Care Planning
Wednesday, June 7 at 6:30 PM
If you suffered a medical emergency or accident and were unable to make your own decisions, who would you want to make them and what would you want them to decide??
While we prefer not to think about it, this could happen to you at any age and at any time. Are you prepared? If not, join us for an information session to learn about:
  • Advance Directives: what they are and why they are important?
  • POLST: what is it and when do you need one?
Individual sessions will be available for those who want additional information and/or assistance completing an Advance Care Directive.
