Program Description
Today's program will begin with Dr. Eshel's presentation of her paper, "The Vanished Last Scream." This paper's point of entry to the psychoanalytic thinking on early breakdown, catastrophic psychic trauma and the last scream is an ancient enigmatic Jewish Midrash which creates an analogy between the voice of a tree being cut down and the voice of a soul departing from the body - a voice that cries out inaudibly from one end of the world to the other. Drawing on the writings of Winnicott, late Bion, and later Eigen, Tarantelli, and Bromberg, Dr. Eshel explores the depths of early breakdown and core catastrophe, where unthinkable horror and agony lie buried unknown, unexperienced, unlived, and unrepresented, and the last SOS scream vanishes.  This underlying catastrophic impact forecloses the very process of thinking, dreaming and analytic reverie. Dr. Eshel contends that the unthinkable cannot be thought, but only relived and gone through with the analyst. Using detailed clinical examples from Bion, Winnicott, and from her own work, the author puts forth a profound form of the analyst's being-there, in-tu-it, interconnecting with the catastrophic impact and the unthinkable horror and agony of the patient's emotional reality, and hearing the vanishing scream. This interconnected being, which may become at-one-ment with the patient's innermost experience, keeps both the scream and the patient's hope from dying out, and gradually creates new experience within.

Following a period of discussion with the audience, Dr. Eshel then will present a brief lecture about the radical influence of Winnicott and late Bion for clinical work with patients suffering from early breakdown, drawn from her paper, " From extension to revolutionary change in clinical psychoanalysis:  The radical influence of Bion and Winnicott," recently published in the December issue of The Psychoanalytic Quarterly .

There will be plenty of time for audience discussion to deepen our learning.

Audience
MD's, Psychologists, LCSW's, MFT's, Registered Nurses, and other learners interested in developing psychoanalytic understanding.

Course Objectives
As a result of this course, participants will be able to:
  1. Differentiate the clinical theories of Winnicott, Bion, Eigen, Tarantelli, and Bromberg regarding the depths of early breakdown and core catastrophe;
  2. Take steps to improve the clinician's capacity to interconnect with the catastrophic impact and unthinkable horror and agony of a traumatized patient's emotional reality;
  3. Utilize the models of Winnicott and late Bion to improve outcomes with patients who are difficult to reach.
