Introducing the Quality Forum Awards!
Connections members can earn awards for achieving Quality Forum performance targets AND for participating in and contributing to the Quality Forum.
Performance Awards
 
As part of the Quality Forum, a health center can earn an award for clinical quality measure performance. Each of the 6 Quality Forum Measure listed below will have three targets, Bronze for Grant Year 2, Silver for Grant Year 3, and Gold for the Healthy People 2020/Connections Quality Forum goal. Each target has a point value. At the end of July, which is the end of the grant year, the points for the individual measures will be tallied to determine the grant year total for each health center. The health centers with the highest and second highest point totals will receive the following awards.

PLATINUM Performance Award
DIAMOND Performance Award
$850 toward the purchase of one to two EHR-supported devices that enhance EHR use will be awarded to the health center with the most measure performance points.
$425 toward the purchase of one EHR-supported device that enhances EHR use will be awarded to the health center with the second most measure performance points.
Citizenship Awards
 
A health center can also earn a Citizenship Award through participation in and contribution to the six Quality Forum meetings held each grant year. At the end of July, the points will be tallied to determine the grant year total for each health center. The health centers with the highest and second highest point totals will receive the following awards.

PLATINUM Citizenship Award
DIAMOND Citizenship Award
KAMU Annual Conference registrations and two nights lodging during the conference for 2 attendees will be awarded to the health center with the most citizenship points.
KAMU Annual Conference registration and two nights lodging during the conference for 1 attendee will be awarded to the health center with the second most citizenship points.

Quality Forum Measures
 
Quality Forum meetings will focus on six clinical quality measures and the use of existing tools to assist with monitoring performance. The Quality Forum Measures of Focus are:
  1. Cervical Cancer Screening
  2. Childhood Immunization Status
  3. Colorectal Cancer Screening
  4. Diabetes Uncontrolled
  5. Hypertension Controlled
  6. Patients Screened for Depression and Follow-up
