If you have lost a family member to suicide, and if you would like to have him or her memorialized on the next memorial quilt, please follow the directions below.

(We display the quilt at events all over Tennessee to promote awareness of suicide, education, and prevention.)

DIRECTIONS

Send the following information to Karyl Chastain Beal at [email protected] or [email protected] along with a photo of the person. (See notes below about the photo.)

1. Complete name of the person to be remembered.
2. His or her birth date
3. His or her death date
4. Where was she or he from, city and state
5. Your name
6. Your connection to the person
7. Your e-mail address
8. Your phone number
9. What city and state do you live in?

PHOTO TO SEND

You need to send in a good clear photo of the person. If other people are in the photo, they will be cropped out. (The photo should be sent as a JPEG file to the same e-mail addresses listed in the directions.)
