E-Rate 470/471 Forms
Funding Year 2017
- Thursday, April 13, 2017 is the last day to file a Form 470 for FY 2017 meeting the required 28-day waiting period prior to submitting a Form 471.
- Thursday, May 11, 2017 is the last day to file a Form 471 for FY 2017. The window closes at 11:59 p.m. EDT.
ONLY applicants that
associated their EPC BEN with the TRIG E-Rate Activity KISD Internet Access and Transport Consortium BEN# 17011660 in EPC and submitted a Letter of Agency (LOA) can use the KISD Consortium Form 470# 170060364 in place of their own Form 470. Applicants that submitted LOAs and were included in the KISD Consortium are: Rockford School District, Bay City Academy, Boyne Falls Public Schools, Grand Traverse Academy, Manton Consolidated Schools, Oscoda Area Schools, St. Johns Public School District, Clinton County RESA, and Ionia Community Library. If these consortia members verify that they are within the Managed Way coverage area and chose to use Managed Way, they may use the above
Form 470# 170060364 in lieu of completing their own Form 470.

The Michigan Department of Education: TOP 10 IN 10 INNOVATION ROADSHOW

Who Should Attend: Counselors, CTE Staff, Curriculum Directors, District MTSS Leaders, ISD Consultants, School Administration, School Improvement Team, Technology Directors, Title Fund Directors
The landscape is changing as Michigan embarks on the journey to become a Top 10 education state in the next 10 years. The Michigan Department of Education (MDE) will be hosting seven events across the state that will demonstrate ways that districts and schools can maximize both their state and federal funding to be inclusive of a well-rounded education that supports Personalized and Deeper Learning. This event will highlight local exemplars and begin the collaborative conversations specific to three key focus areas: well-rounded educational opportunities, safe and healthy students, and effective use of technology.
Join us in this collaborative effort between MDE's Offices of Education Improvement and Innovation, Field Services, Career and Technical Education, School Support Services, along with our partners through the Great Lakes Stewardship Initiative, Math and Science Centers Network, and the Technology Readiness Infrastructure Grant at one of the following locations.

- May 4, 2017 - Kalamazoo RESA
- May 11, 2017 - Innovation Zone at the Genesee Career Institute
- May 15, 2017 - Mecosta-Osceola RESA
- May 16, 2017 - Oakland Schools

- May 31, 2017 - Cheboygan-Otsego-Presque Isle ESD
