SIM recruits 150 providers to join circle of healthcare innovators
The Colorado State Innovation Model (SIM) released its application for cohort 2 and hopes to recruit 150 additional practices for the initiative. We Encourage practices to apply today: bit.ly/2kUdsHe
 
The SIM initiative, which is funded by the Centers for Medicare & Medicaid Services, is helping 92 practices in Colorado integrate behavioral health and primary care and test value-based payment models.   • March 31, 2017 - Practice RFA closes at 11:59 PM
  • June/July 2017 - Selected practices notified 
  • September 2017 - Cohort 2 practices kick-off
