January 2017 HHQI Agency of the Month

This month, HHQI recognizes Community Nursing and Home Health of Johnstown, Pennsylvania for their dedication to improving home health care quality.

Home health agencies that are active in the HHQI National Campaign and closest to the  national, risk-adjusted ACH 20th percentile and/or the national, risk-adjusted Oral Medication Improvement 80th percentile (or better) are eligible for random selection as HHQI Agency of the Month. 

Be in the spotlight.  Nominate your agency today , and if you are selected, your organization will be prominently featured here in HHQI Inside Edition -- which reaches more than 15,000 subscribers -- and on the HHQI website. Your achievement will also be shared via HHQI's social media channels.
HHQI Pilot Study Featured in Home Healthcare NOW

Home Healthcare NOW recently published an article written by several members of the HHQI team:  Janet Lynch, PhD, Corporate Science Officer; Cynthia Sun, MSN, RN Project Coordinator; and Stacie Deslich, MSHA, Health Data Analyst.

Integrating Preventive Cardiovascular Health Into Home Health: A Pilot Study  describes a pilot study conducted in conjunction with the launch of HHQI's cardiovascular health initiative. The project aligns with the national Million Hearts® initiative to prevent a million heart attacks and strokes. The data for the study was drawn from HHQI's Home Health Cardiovascular Data Registry (HHCDR) where agency's submit a few pieces of additional patient information related to one or more of the measures that focus on aspirin as appropriate, blood pressure screening, cholesterol management, and/or smoking cessation.

The study demonstrates that home healthcare agencies can contribute efforts with the national initiative to reduce heart attacks and strokes. The results supported the expansion of the HHCDR nationwide which is now open to all Medicare-certified home healthcare agencies.

New HHQI Resource Library Coming in January 2017

The HHQI National Campaign website is loaded with hundreds of free, high quality resources for home health agency leadership, nurses, therapists, social workers, aides and patients. However, some participants have expressed that finding resources can sometimes be a challenge because they are spread across many pages of the website. In response, HHQI will be launching a new consolidated Resources page in January 2017.

The new online resource library will be a one-stop shop for all HHQI resources and easily accessible via the main navigation menu across the top of all pages of the website. Filters for Best Practice Intervention Package (BPIP), topic, material type, audience and language will make it easy to quickly sort through hundreds of HHQI's resources and find exactly what you need. 

As soon as the new resource library is available, we will make an announcement via our social media channels. Use the links at the bottom of this newsletter to follow us! You will also be notified by email if you subscribe to any of our mailing lists.

If you're already familiar with where certain resources can currently be found on the website, don't worry. You will be able to continue accessing those resources on those pages. The new Resources page will supplement existing resource pages, not replace them.

In order to make the most of your limited time, HHQI's three primary online Networking events -- LiveChat, Cardiovascular Learning & Action Network (CardioLAN) webinars, and Underserved Populations (UP) webinars -- will  each be held quarterly. 

Events will rotate to always begin at  2pm (ET) on the third Thursday of the month.

January 2017 QIO News: 2016 CMS Quality Conference

The 2016 Centers for Medicare & Medicaid Services' (CMS) Quality Conference brought together over 2,400 stakeholders from across CMS, the Department of Health and Human Services, CMS Quality Networks, health care providers and beneficiary advocates to discuss how to better promote collaboration and innovation to improve health outcomes.
