You spoke, and we listened!!   In order to streamline the re-registration process, REGISTRATION FOR THE 2017-2018 SCHOOL YEAR WILL BE COMPLETED ONLINE (with the exception of the Bus Transportation and State Book Form attached to this email).  

Please follow the following instructions to properly register for Kindergarten:

 A Medication Information Form must be completed if there is a specific health issue which may require medication to be administered during school hours.


New Jersey Residents: 

 Please complete a Transportation Application for each of your children, regardless of whether you plan to use the bus or not.   Please fill out the form and return it electronically or personally to the school office.   

Please complete a NJ TEXT BOOK FORM.  Also complete and sign the bottom portion of the Textbook Form. Please fill out the form and return it electronically or personally to the school office.    

New York Residents: 

 Busing is provided by your individual Local School District. Selected District forms are listed below. If your district is not listed please contact your district for information and transportation forms for the upcoming year.  All forms should be completed and returned to them.

Current Physical & Immunization Record:  A current physical form and immunization record are mandatory for all students entering these grades.  Please submit your physician's universal form to the Health Office no later than July 13, 2017 All health records must be complete in order for your child to start school in September.  If you have a current universal health form you may upload it and attach it to your Registration form.

Tuition:  Tuition rates for the 2017/2018 School Year will be announced in the near future and may be paid in either one or ten payments. FACTS Tuition Management will maintain all of our tuition accounts.  Please visit the OLMA website  www.olmacademy.org and click on FACTS on the left sidebar for detailed information. ALL NEW FAMILIES MUST REGISTER WITH FACTS in order their registration to be complete.

Fee Information:  Registration fees ($175 per student and H.S.A. dues $35 per family) should be paid in one check and submitted upon completion of your Registration form to the School Office.   All 2015/2016 tuition/fees MUST be up-to-date in order to re-register.  Technology Fees ($60 per student), 8th Grade Graduation Fees ($150 per student), and Out of State Text Book Fee ($60 per Out of State student) 
will be billed with your tuition.

OLM parishioners must return the Parishioner Verification Letter sent by Fr. Charles along with your completed registration forms.  Parishioners of all other parishes in the Archdiocese of Newark must have their pastor complete a Verification of Active Parishioner Status (available in the School Office.)  These letters will determine your tuition rate.

KINDERGARTEN:  ALL FORMS AND REGISTRATION FEES ARE DUE IN THE SCHOOL OFFICE NO LATER THAN FEBRUARY 3rd.
