Reducing Hospitalizations Among "Super-Utilizers" - The Care One Clinic
Presented by
:
Robert Leverence MD, Deepa Borde MD, Jacqueline Pinkney LCSW, Joy Wright PharmD, and Robin Rocks RN
University of Florida Health officials detail how they cut hospitalizations and reduced uninsured hospital admissions-without hurting the system's bottom line. In an effort to better coordinate care for its super-utilizers, UF Health created an "ambulatory intensive care unit" called Care One Clinic in order to connect patients with primary care.
The clinic, housed at UF Health Shands Hospital, is staffed by:
- A social worker, who performs needs assessments for patients in their initial visits to identify barriers to care, such as low health literacy or a lack of transportation;
- A clinical pharmacist, who reconciles medications, provides patient education, and when necessary monitors opioid use;
- A hospitalist and an addiction specialist, who assist with condition management, address untreated needs, and provide referrals; and
- A nurse, who helps provide care.
Among the super-utilizers, hospitalizations dropped by 25 percent, hospital days fell by 23 percent, and ED visits decreased by 11 percent in the six months after enrollment, compared with the six months before.
