The BUILD Health Challenge Joins All In!

All In is pleased to welcome the BUILD Health Challenge into its network of networks. The BUILD Health Challenge is a funder collaborative that encourages communities to build meaningful partnerships among hospitals and health systems, community-based organizations, their local health department and other organizations to improve the overall health of local residents. BUILD Health communities are implementing bold, upstream, integrated, local, data-driven solutions aimed at influencing the social determinants of health. Project Spotlights

This month, we're featuring two projects that are fostering multi-sector partnerships to better coordinate social and medical services for high-risk patients. 

CHP: University Hospitals Rainbow Babies & Children's Hospital

As a part of the CHP Program, University Hospitals Rainbow Babies & Children's Hospital (UH Rainbow) is implementing the Integrated Data Evaluating Asthma Risk in ChildHood (IDEA RICH) initiative. IDEA RICH is integrating clinical health system data with a longitudinal database of community factors.This vision is to make analysis of these data available to a community partnership in order to collaboratively reduce the impact (burden) of childhood asthma among children to whom UH Rainbow provides primary care in specified low income neighborhoods in Cleveland. IDEA RICH intends to build partnerships that prove foundational for future health improvement activities.The use of nuanced contextual data and the inclusion of a variety of sectors, including health care, public health, housing, social services, environment, and criminal justice offer hope to create a culture of informed accountability to enhance the health and health care for vulnerable populations of children in Cleveland. 


DASH: Maine Data Across Sectors for Health

Through DASH, HealthInfoNet is creating a care management system that integrates health care and social services data to help providers better understand the needs of patients. HealthInfoNet is leveraging the state health information exchange (HIE) to integrate electronic health record data from three critical access hospitals and seven federally qualified health centers with data on social factors like housing, medical transportation, and child services from two community action agencies. Once organizations are connected to the state HIE, care managers will be able leverage HealthInfoNet's predictive analytics system to identify the most complex, high-need patients and intervene appropriately.


Funding Opportunities

The BUILD Health Challenge

The BUILD Health Challenge just announced their next wave of funding opportunities. With five new funders this year, BUILD is scaling its upstream approach to broaden impact. In addition to funding, awardees will gain access to a robust array of coaching and support services including, but not limited to, web-conferences, learning labs, coaching, training, communications, and evaluation support.


Bridging for Health

Collaboratives across the nation are making bold strides to improve the health of their communities. Are you part of one?  Share your experience! This fall, the Robert Wood Johnson Foundation's Bridging for Health initiative is looking for additional sites to join their cohort that are improving community health through innovations in financing. Complete an interest form to determine if your collaboration is a good fit.
