Dear 7th and 8th Grade Student-Athletes and Parents/Guardians,

Tryouts for the Winter II season begin on
Tuesday, January 17, 2017
Medical forms (physical & medical update)  are now being accepted by the school nurse.

On Thursday, December 22 student-athletes will have a preseason meeting with their prospective Winter II coaches. Meeting locations will be posted in the cafeteria on Wednesday, 12/21.  At this meeting the coaches will discuss tryouts and team expectations. The late buses will run at 3:30 pm. Student athletes may use the late bus or arrange for their own transportation for after the meeting. 

All student-athletes trying out for a winter II sport must have the following documents completed and submitted in order to participate. (Click on the form name for the document.)
  1. School Physical - Must be completed on a Roslyn medical form. All physicals must be dated April 1, 2016 or later. This form must be returned to the middle school nurse. (The high school and middle school forms are the same.)
  2. Medical Update - completed by parent/guardian after December 17, 2016. (form given to nurse)
  3. Parent Consent - all parts of the form must be completed by the parent/guardian each season granting permission for your child to tryout/participate in a particular sport. This form must be returned to the coach by the first day of tryouts in order to be eligible to participate. (form given to coach by the first day)
  4. Code of Conduct - must be read and signed by the student-athlete and  parent/guardian. This  document must be signed and returned to your son/daughter's  coach by the first day  of tryouts. (form given to coach by the first day)
  5. Concussion Information Permission Form - A concussion is a type of traumatic brain injury caused by an impact or blow to the head or body. Concussions can happen in any sport, please read and sign the attached form. (form given to coach by the first day)
  6. Impact Concussion Management Test - All student-athletes participating in the athletic program must take the Impact Concussion Management Test. Many students have already taken this test during the school year. Please contact the athletic office if you have any questions
  7. Vision Clearance Form (if applicable) If your child's vision screens at 20/200 or     worse, a vision clearance form is also required for contact sports.
The above requirements must be completed in order for your  son/daughter to try out for a winter sport. No exceptions.

FORMS ARE NOW BEING ACCEPTED BY THE SCHOOL NURSE.
