To our HealtheConnections participants:

I am proud to share with you the progress HealtheConnections has made through September this year. Our data is in and the results are incredible! The numbers speak for themselves: This works. The HIE is growing and we have you, our partners, to thank for it.

We’ve documented some huge successes year-to-date, and this means great things for all of us. With more usage and participants involved, everyone can get more out of the HIE resources.

If you have friends or colleagues who have yet to connect with us, point them our way. We’re here to help everyone make the most out of our free and secure resources. Faster patient visits, fewer calls for records, easier appointments – our goal is healthy patients and happy staff throughout central New York.

Once again, thank you for letting HealtheConnections be part of your operation. I can’t wait to see what the coming months have in store.

Got Data? Share it with the HIE!

As one of the key goals for 2016, HealtheConnections has concentrated on data acquisition in order to populate the health information exchange with more pertinent patient data. HealtheConnections is aiming to reach 200 data sources by December 31, and we're almost there with 167 total participants sending data to the HIE.

When our participants send patient data to the HIE, they help to build a more complete record for each patient. Increasing the number of data sources submitting lab results, radiology studies and reports, transcribed reports and other patient information enriches the data available to connected healthcare providers and professionals.

NYSDOH Data Exchange Incentive Program Eligibility Expansion

The Data Exchange Incentive Program (DEIP) offers funding to help offset the cost of connecting electronic health record (EHR) systems to HealtheConnections.

Are you eligible? 

Nineteen of HealtheConnections’ participating organizations - representing 515 providers - are currently enrolled in this program to receive $700,000 in incentives. These 19 organizations use nine different EHR systems which are currently integrated with HealtheConnections: Chartmaker, eClinicalWorks, EPIC, GECentricity, Inforia, Medent, Netsmart, NextGen, and Vitera.  The original program launched in 2014, funded through NY State Department of Health with monies received from Centers for Medicare and Medicaid Services (CMS),  requires participating organizations, that service a minimum of 20-30% Medicaid patients are are Meaningful Use Stage 1 eligible, to send a robust set of patient encounter information to HealtheConnections.

The program has been expanded to include NYS Regulated Articles 28, 36 and 40 Facilities (Nursing Homes, Diagnostic and Treatment Centers, Certified Home Health Care Agencies and Long Term Home Health Care Programs, Hospice), Behavioral Health Providers, Medicaid eligible Providers that accept Medicare and Medicaid eligible Providers.

Announcing Our Newest Population Health Initiatives
This summer, HealtheConnections was awarded and began work on two projects sponsored by the New York State Department of Health, with funding originating from the Centers for Disease Control and Prevention: 

HealtheConnections and Kinney Drugs, a regional community pharmacy, are piloting several complimentary activities to improve hypertension and diabetes self-management in our community.

Kinney pharmacists at four locations in Cayuga, Madison, and Onondaga counties will educate patients in the use of home blood pressure self-monitoring devices and management of their hypertension. These pharmacists will also provide enhanced education on diabetes self-management and referrals into community-based Diabetes Self-Management Programs (DSMP) and health care based Diabetes Self-Management Education (DSME) classes.

Additionally, Kinney prescription data in the health information exchange (HIE) will be used to alert providers about medication non-adherence in their patients, allowing the providers to work closely with Kinney pharmacists to address barriers and improve health outcomes. 

HealtheConnections is working with local organizations in Syracuse to make opportunities for healthy eating and active living more accessible and inclusive of people with physical and intellectual disabilities. The NYSDOH Disability and Health Program will collaborate on this work, providing technical assistance to local organizations about how to use adaptive equipment and integrate the perspective and needs of people with disabilities into public health program planning. 

HealtheConnections is also involved in a third project, which is being led by St. Joseph’s Hospital Health Center and funded through Trinity Health: 

This initiative is focused on policy, systems, and environmental changes that address the underlying causes of chronic disease in several high-needs neighborhoods in the City of Syracuse. St. Joseph's Hospital Health Center is partnering with the Onondaga County Health Department, Near Westside Initiative, Northside Urban Partnership, The Lerner Center for Public Health Promotion at Syracuse University, and HealtheConnections in these efforts. HealtheConnections' role is to evaluate the local strategies, to measure their impact and ensure their effectiveness in our community.

DSRIP 2017: Lessons Learned and Paving the Way for Success (Dec. 6):
HealtheConnections President & CEO Rob Hack will join a panel of speakers to discuss  some of the critical challenges—and solutions—that lay ahead as the implementation and optimization of the DSRIP program continues forward.
HealtheConnections Sponsors Making Strides Against Breast Cancer Walk 
For the second year in a row, HealtheConnections sponsored the Start Line for the Making Strides Against Breast Cancer. The walk was held on Sunday, October 16 to raise awareness and money for breast cancer research.  

This year the walk relocated to the SRC Arena on the OCC campus. Over 4,500 people attended the event. One lucky walker who stopped by the HealtheConnections table was awarded a gift card to Mirbeau Inn & Spa after correctly guessing the number of pink and white candies inside a jar. (There were 938 candies, and she guessed 925!)

Everyone who visited the HealtheConnections table received a pair of pink sunglasses to wear for the walk around the campus, the perfect accessory for what turned out to be a sunny fall day!

Overall, $382,500 was raised for the American Cancer Society. 
Welcome to Our New Employees: Karen Gonyea and Shruti Kedia

Karen Gonyea, Operations Support Specialist, has been an Administrative professional for over 20 years. She has previously worked as a program assistant supporting several different health programs. Gonyea earned her associate’s degree in Computer Information Systems from Onondaga Community College.

Shruti Kedia, HIE Integration Analyst, has over five years of experience in providing clinical care. She has worked with healthcare data over the course of various projects in a multitude of settings, including an internship with the Rochester RHIO. She completed her Bachelor of Dental Surgery from India, where she practiced as a dentist before earning her master’s degree in Biomedical Informatics from Columbia University.
