Join us for the
Data! Fostering Health Innovation in Kentucky and Ohio
forum as we showcase novel and effective uses of health data in our region.
The morning keynote speaker will be Aaron Truchil, MS, director of analytics and informatics at the Camden Coalition of Healthcare Providers. He will discuss how the coalition shares person-level data across sectors and builds community partnerships to drive hotspotting and whole-person care in Camden, N.J.
The afternoon keynote speakers will be Director Barbara Sears, Ohio Department of Medicaid, and Dr. Gil Liu, medical director of the Department for Medicaid Services in Kentucky. They will share the ways these Medicaid programs are using data to improve health.
Local entrepreneurs, researchers, agencies and organizations will share the latest innovations to improve population health in our communities. We hope this exciting and fast-paced forum will inspire you to harness the power of health data to foster change in your community.
Health data enthusiasts, nonprofit and philanthropic organizations, entrepreneurs, hospitals, health departments, community planners, health advocates and researchers are encouraged to attend.

Kentucky and Ohio's Healthy Eating and Active Living Policy Forum

Please join the American Cancer Society for a luncheon panel discussion on the research linking healthy eating and active living environments with cancer risk, as well as treatment outcomes and patient quality of life. The forum will showcase evidence-based initiatives and potential public policy solutions for addressing obesity and barriers to nutrition and physical activity in Ohio and Kentucky.

Integrating Cognitive Behavioral Skills Into Clinical Practice
November 23 2017, Erlanger
Psychosocial risk factors, such as fear of movement and pain catastrophizing, have a negative infuence on rehabilitation outcomes and often result in higher levels of pain and disability. Changing Behavior through Physical Therapy (CBPT) and Occupational Therapy (CBOT) is a program designed to improve patient outcomes by addressing specifc psychosocial risk factors. This program has been approved for 7.5 contact hours in KY for PTs and OTs.

2nd Annual Children's Environmental Health Summit

Fuel Up to Play 60
Jump Start Healthy Changes Grants offer funding for schools to improve opportunities for healthy eating and physical activity. Funds are awarded for schools to implement at least one Healthy Eating Play and One Physical Activity Play from the Fuel Up to Play 60 Playbook.
Eligibility: Schools (K-12) that are enrolled in the Fuel Up to Play 60 program for the 2017-2018 school year and participate in the National School Lunch Program are eligible to apply.
Amount of funding: up to $4,000.
Deadline: November 1, 2017

America Walks Community Change Micro Grant
America Walks is excited to extend another round of community change micro grants to individuals and communities interested in improving the public sphere for walking or otherwise helping to make walking more commonplace.
This program awards up to $1,500 to fund or help to catalyze smaller-scale, low-cost projects and programs that increase the prevalence of walking, expand the diversity of people and organizations working to advance walkability, and help to make walking safer, easier, and more fun for all community members.
Deadline: November 10, 2017

The Department of Agriculture's Community Food Projects Competitive Grant Program
The program is designed to, among other things, meet the needs of low-income people by increasing their access to fresher, more nutritious food supplies; increase the self-reliance of communities in providing for their own food needs; promote comprehensive responses to local food, farm, and nutrition issues and create innovative marketing activities that mutually benefit agricultural producers and low-income consumers.

Rural Health Care Services Outreach Program
This community-based grant program is aimed towards promoting rural health care services by enhancing health care delivery in rural communities. Outreach projects focus on the improvement of access to services, strategies for adapting to changes in the health care environment, and overall enrichment of the respective community's health.
