It’s serious and can be uncomfortable, but it’s one of the most important things any of us will ever do.  Advance Care Planning helps your loved ones advocate for you if you cannot speak for yourself, and protects them from having to make hard choices about your care without your guidance.  Advance Care Planning includes the Advance Healthcare Directive and may include the POLST form.  Randy Reitzes, LVN, Nurse Educator for Hospice by the Bay, will explain both of these documents in depth with time for questions and answers.
