At the University of Maryland, we want to make sure that all our students are healthy and covered. Therefore, all new students have to meet certain health requirements in order to enroll at UMD. Please read the instructions below carefully to make sure you satisfy all requirements of the University Health Center.  Failure to complete these items may result in a block on your account (inability to register for classes, etc.).  But more than that, it will also likely save you time and money if you can prepare for these requirements prior to your arrival to UMD.  
Today's Tip: How to complete the Immunization Form and other important Information from our Health Center

IMMUNIZATION FORM OVERVIEW
  • The Health Center requires you to submit the completed form below (in English) no later than August 25, 2017 or you will receive a "registration block," meaning you will be unable to register for courses or adjust your course schedule.
  • Please submit the forms online- the earlier the better!
  • The Immunization Form is available below and must be completed in English. This form must be submitted before orientation to the University Health Center online

Step 1: Complete Top

Complete the personal information in the section at the top of the form (your name, UID number, date of birth, etc.). 

 Step 2: Immunization for Measles, Mumps and Rubella (MMR) - Section A

The University of Maryland at College Park requires ALL incoming students to provide completed proof of immunization dates for Measles, Mumps and Rubella (M.M.R., 2 doses), and Tdap/TD within the past 10 years. A copy of a lab titer for Measles, Mumps and Rubella will be accepted in lieu of the immunization dates. 

Step 3: Meningococcal (Meningitis) - Section B/C

Required if you will be living in on-campus student housing. Alternately, you can sign a waiver opposing the vaccine (Section C).
   
Step 4: Tuberculosis (TB) Testing Questionnaire - Section D
 
Every student must complete this section. If you have answered yes to any of the questions, the Quantiferon Gold or T-spot must be done. This will be only accepted if done in the US.

Step 5: Optional Recommended Immunizations - Section E

Other recommended immunizations for all incoming students are: Chicken Pox (Varivax; 2 doses); Hepatitis A (2 doses); Hepatitis B or Twinrix (3 doses); HPV (3 doses); Influenza (yearly). They are not required for students at the University of Maryland. These immunizations can be acquired through the University Health Center, if desired, at a later date and by appointment.

Step 6: Option Gender Identity - Section F
This section is optional, for students that want to more specifically list their gender identity.

Step 7: Submission of Completed Documents
Immunization forms must be submitted to the University Health Center before orientation. Forms may be uploaded to  www.myuhc.umd.edu(link is external). You will be prompted for a Directory ID and Password, then you will enter your UID number.  Go to forms, select Immunizations, and enter all the appropriate requested immunization dates found on your records.  Click on Add Immunization Record - where you will scan your document to the record.  Then Submit Final.

Appointments/ Final Information:

If you require an appointment with the Health Center, please set one up via iTerp.  This can be found under Session Sign-Ups- Health Center Appointments. 

Students who receive an email from Health Center staff stating that an appointment is required should follow up by making an appointment. Do not make an appointment unless you have received an email from the Health Center staff stating that you need one.

Appointments will be offered every hour from 9am-11am and 1pm-3pm on August 17, August 18, August 21, August 22, and August 25. Each slot will be limited to 75 students so please sign up as soon as you receive an email from the Health Center requesting to set up an appointment. 


Please be especially careful about the Quantiferon Gold blood test. In many instances, it is required that this test be done in the US. If you do not intend to enroll at UMD and would like to stop receiving these emails, please inform the admissions office (undergraduate students) or the Graduate School and your academic department (graduate students).
