  As we implement the Colorado State Innovation Model (SIM)
and collect data from cohort 1 practices and community mental health centers that are integrating behavioral health and primary care, the SIM team will share actionable information and use the data to influence future SIM practice cohorts.

The team is preparing to release its application for cohort 2 this winter and will start to publish actionable information about what it takes to integrate care to help stakeholders learn from the SIM initiative, which is funded by the Centers for Medicare & Medicaid Services. 

SIM creates powerful partnerships

This short video features many of the faces and voices of people who have been integral in creating, supporting and furthering SIM's mission to integrate behavioral health and primary care. They talk about why it matters to healthcare providers, patients and insurance companies that are working with SIM to create sustainable pathways to integration through value-based payments.

SIM weighs in on national initiatives

The Centers for Medicare & Medicaid Services (CMS) recently released  a request for information  to solicit stakeholder feedback on state-based payment and delivery system reforms to help shape future federal-state initiatives, and the Colorado State Innovation Model (SIM) submitted recommendations Nov. 4. 

CMS is inviting comments on concepts for a potential future state-based initiative that would help states implement broad-scale, multi-payer delivery and payment reforms that have a clear vision for multi-payer alternative payment models (APMs) that include Medicare and focus on the health outcomes of the entire population of a state through alignment of care delivery and payment. Recognizing that there are multiple pathways to achieve this vision, CMS is asking for public input on ways to help states develop the operational and infrastructural capacity needed to implement a multi-payer model that includes Medicare and could be an APM under MACRA. The SIM office submitted general responses that reflect the team's experiences with SIM implementation.
ACC Phase II request for proposals

Stakeholders are invited to comment on the Accountable Care Collaborative Phase II draft request for proposals through Jan. 13, 2017. 

Phase II of the Accountable Care Collaborative (ACC) will leverage the successes of Health First Colorado's programs that enhance member and provider experience, improve member health and life outcomes and use state resources wisely.  Participate in a webinar Nov. 10 and learn more about in-person meetings across the state that will be scheduled this month: Colorado.gov/HCPF/ACCPhase2

Lessons learned from SIM cohort 1

Learn more about lessons from the first cohort of practices that were accepted into the Colorado State Innovation Model (SIM) with a SIM team member, who will provide an overview of the initiative and a practice transformation coach, who will discuss some of the barriers to the integration of behavioral health and primary care and successful workarounds.

Help shape telehealth in Colorado
