Hello Division Constituents and Community Partners,
 
With the Thanksgiving holiday close at hand, I want to thank the thousands of individuals, families, and professionals who partner with this Division to increase opportunities for individuals with intellectual and developmental disabilities, and extend our warmest wishes for a happy and safe holiday.
 
I am also pleased to share some important Division announcements with you, which I encourage you to forward to anyone who may benefit from the information.

If you haven't already, please mark your calendars for the Division's  Quarterly Update Meeting for Families and Providers, scheduled for  Tuesday, December 6 (Providers:  2:00 - 4:00 PM; Families:  5:00 - 7:00 PM). Having received positive feedback from the first quarterly update meetings, held in September, we are looking forward to once again having the opportunity to share important Division news directly with families and providers.
 
Registration for the quarterly update meetings is required and will be taken on a first come, first served basis.  Registration details will be distributed early next week.


 
 
The searchable online database of providers of DDD-funded services and supports is now open and available for use at  https://irecord.dhs.state.nj.us/providersearch. The Provider Search Database enables users to search for all types of providers by a number of different criteria, including provider name, location, and service. Instructions for conducting a search are found at the top of the Provider Search Database web page. 
 
To search for  Support Coordination Agencies, go to the Provider Search Database ( https://irecord.dhs.state.nj.us/providersearch) and follow these four steps:  (1) under Filter, select "Service" and check Support Coordination;  (2) select "Medicaid Approved" and check the box;  (3) select "County Served" and select the county in which the individual resides; and  (4) click the magnifying glass. 
 
DDD will accept update requests from DDD/Medicaid-approved Supports Program providers who wish to make updates/changes to their Provider Search Database information. To request changes, Supports Program providers must submit the SP Providers Update Request form to the Provider Database Help Desk at  [email protected] 
 

 
Community Services Structure and Contact Information

As previously announced, the Division has re-structured Community Services to reflect the external system reforms currently underway. While the Division continues to maintain nine Community Services Offices that  serve all 21 New Jersey counties, statewide units have now replaced the former regional structure. Please see the  Community Services Structure  table for a list of directors and their contact information.
